I understand that this is a nursing home and it will be hard to keep down the smell of urine. My cousin bathroom has had bowls that’s in toilet due to be stopped up my relative is non mobile, he had suffer a stroke and aneurysm, the staff some or helpful and others trying to find some where to hide. And I have called the rehabilitation center several times no one ever answers. We live out of town so if we wanna talk to someone we have to go up. I read the comments on this page stop lying this place is awful and half the staff there are young adults this type of work you have to have a heart to do and this generation ain’t cutting it.

My older brother has severe cognitive impairment and, after a number of falls at home, had to move to a place where he can get around-the-clock care.
He had open heart surgery in 2021, and did his rehabilitation at South Health and Rehabilitation (SH&R). We were impressed by the level of care and attention he received back then. When we were given the list of facilities for this most recent medical issue, we were happy to see SH&R listed. He was quick to say, “I like that place and the people that work there. They were so nice to me. I want to go there.”
From the very beginning, the staff made the process as easy for us as they possibly could. I was especially grateful to Shanetrice Fortner in Admissions for being so thorough in explaining everything to us. We are able to make informed decisions about my brother’s care and ongoing needs, and this has been the case since Day One (May, 2025).
They always let me know if there are important issues that need to be discussed, and it had taken away so much worry I’ve had concerning my brother’s safety.
Granted, this is not a five star hotel. But it’s the best managed nursing home facility I’ve ever seen personally. And the food prepared for the residents is very appealing! My brother says he enjoys every meal. Most importantly, the staff there are kind, compassionate, professional and communicative. I tell them regularly how grateful we are to have them caring for my brother.
I highly recommend them.

My aunt was transferred here from UAB hospital. I guess this was the only place that would accept my aunt because of financial reasons. I called to inquire about her and the lady on the phone was beyond rude and disrespectful. I was just calling to check on my aunt who was supposed to arrive today and I told them I had talked to a nurse practitioner and they couldn’t figure out who I had spoken to then they said”oh you mean that white lady” which how am I supposed to know if I’d only spoken on the phone? As a white person myself I found that to be a little unprofessional. But then they transferred me to a different lady and I asked about my aunt. I was personal with her and told her my aunt and me arent really on speaking terms because she’s upset because she thinks I’m in the reason she’s in the nursing home. All I wanted to know is how my aunt was doing and if she was open to seeing me. The lady very rudely “well what do you want me to do do about it”? She then started yelling and “offering” to go ask if my aunt wanted to see.me but this was done in a very hateful and aggressive tone that brought me to tears. I couldn’t do anything but say “no that’s okay” and then she yelled at me said “Mhmmm that’s what i thought. You’re welcome.” This interaction was enough to cause a mental health episode as I am autistic and borderline personality disorder. I cried in my room for hours and now I’m worried my aunt will be treated terribly as well and I don’t know to help her !!

If you care anything about your love one you would not send them to this dump. This place is horrible Tyler the administration is a compulsive liar and will tell you anything just to cover all the wrong that’s been done. Your family will not get the care they need. The workers are sorry and have nasty attitudes. The nurses have poor work ethics. Ebony, Penelope, Angela, are some of the nurses that work there and they purposely ignore the needs of the residents. It’s a wonder they even are still open. They too will be close eventually the reputation is horrible. Staff absolutely horrible. Food is disgusting flies and knats are taking over the place. Just know your concerns and needs will go unnoticed your family member will not stand a chance here. Remove them immediately……..if you care about them. Sorry excuse for being a rehab. You drop them off on Monday they gone on Tuesday. Family members clothes went missing. Hours and hours no check on residents. My family member had missed meals and lost a great amount of weight cause no one was feeding her.

I want to give this place zero stars but we don’t have that option. This is a very disgusting facility, it always smells of urine and waste my loved ones room is always nasty. I always bring Clorox wipes and spray with me to clean her area. The staff is never around or sitting at the desk making personal calls. The other night I came and my loved one was in pain.. I couldn’t find a nurse or Cna and once I did she stated she gets her pain meds at 7 and she didn’t even realize it was 7:30, who knows how long these clients suffer. I also noticed the clients who can’t feed themselves get NO time or the attention they need. I’m working really hard to get this place shut down I’m not really sure how they even pass inspection.. you can never get in touch with anyone the phone just rings. This particular career path is a ministry and they should do extreme interviews to make sure they are hiring compassionate staff. I am truly disappointed ☹️ I live out of state and I’m always concerned for my grandmother that she isn’t getting the things she needs

How to read this page
Star ratings vs. Care Quality Score: The Google star rating reflects visitor and family opinions collected from public reviews. The Care Quality Score is an independent metric derived entirely from federal inspection data — the two measure different things, and comparing them side-by-side gives a fuller picture.
Where this data comes from
All clinical data on this page originates from the Centers for Medicare & Medicaid Services (CMS), the federal agency that certifies and inspects every Medicare- and Medicaid-participating nursing home in the United States. Reviews are imported verbatim from public Google business listings.
