I would not recommend the rehab facility to anybody's loved one if you really want them to progress. Everything was decent to say the least until the very last day my mom was there. They left her sitting in her feces, on her hands, and still on the toilet which is absolutely unacceptable. I guess they felt like since it was her last day they could finally stop working? My mother did a 20 day stay here. I was up there just about every day to visit her to make sure she wasn't getting treated unfairly and clearly that's the only reason they did their job. Not to mention, the care plan they Rehab director and Social services director called me about didn't go as planned. It was as if my mom was making progress, but in all honestly it doesn't seem like they did much to help her progress because she is worse than she was when she transported there. Home health even stated that themselves during their evaluation. She can barely stand and hold her balance. I was told they would help push her to begin walking with her walker again and apparently that never happened. The social service director did not get things in order to make sure her discharge went smoothly, in fact, I had to remind her that she stated it was ok for her to be discharged a day prior. No equipment had been ordered, they left things off the order which put me at an inconvenience with my work schedule, ordered a walker which wasn't requested because again, she had her own and failed to follow up. I also didn't get a call from home health until two days after she was discharged which again put me at an inconvenience. I been running around like a chicken with my head cut off because things on Aspire end could have been handled a lot more professionally.

This is the SECOND review about the facility. You all have dropped the ball AGAIN!! My sisters and I have been waiting on the finance director or whoever it is that submits applications to Medicaid. Funny how after the first review I left, you all called immediately apologizing about how a medical emergency was handled, yet promising to keep us abreast of the status of the supposedly submitted application. Emails, calls, and texted have all been ignored. WHY? You all are trying to save face, but that’s not working!! Now all of a sudden you all don’t have availability any longer, yet, not informing us. WOW is all we can say. Whomever it is that’s in charge of handling submission of said application needs to contact the family immediately l, let alone, tell us that his spot is no longer available. Again, another disappointment from the facility that didn’t inform us of his medical emergency…SMH!! Y’all don’t deserve any stars!!! And I’m sure you all know his last name from mine.

About this data
How to read this page
Star ratings vs. Care Quality Score: The Google star rating reflects visitor and family opinions collected from public reviews. The Care Quality Score is an independent metric derived entirely from federal inspection data — the two measure different things, and comparing them side-by-side gives a fuller picture.
Where this data comes from
All clinical data on this page originates from the Centers for Medicare & Medicaid Services (CMS), the federal agency that certifies and inspects every Medicare- and Medicaid-participating nursing home in the United States. Reviews are imported verbatim from public Google business listings.
