My mom fell and broke her hip November of 2025. She went to Jacksonville Health and Rehab for 20 days. Mom was well cared for and received excellent therapy to help her gain her strength back.<br>So many of the care givers too time to show her attention. Kim on her first evening made sure she had supper and heated it for her. She helped mom settle in.<br>The facility was clean and no odors were there. The meals were adequate and she requested boiled egg for breakfast and they made sure she received one each day.<br>We were there over thanksgiving and mom was able to leave and join us at home for dinner. He holiday decorations were lovely and there was a very homey atmosphere.<br><br>Our family would highly recommend their service. Five stars.

My aunt has been staying here temporarily while she recovers. I walked in & immediately noticed it not only looked so clean & pretty it also smelled the same.i have been in several places like it about a half dozen & I worked at a group home this place is cleaner than any other I have visited or worked but most importantly the staff are so kind and on top of treating their patients I witnessed while drinking coffee residents not only know their staff well they like them and act like a family this is definitely THE place I would want any of my loved ones or myself if necessary

I had the "pleasure" of being a patient at the Jacksonville Nursing Home and Rehabilitation Center recently for a few weeks. Under the circumstances, the facility was quite pleasant. The nursing staff, techs, dietary staff, custodians, activities directors, nurse practitioner, doctor, but ESPECIALLY the physical therapist MEGAN, ASHLEY, MYKEICIA, CHARLOTTE, JILL, AND JeRON!!!!!! This group of folks treated me with dignity and respect, and even serenaded each and EVERY time I showed up. Hopefully I didn't fail to mention anyone or any area. You all were fantastic and will be in my family and heart forever!!!! REMEMBER, - SORRY MZ. JACKSON, I AM FA REAL 🙄😂😘

So disappointed. Family was trying to get two family members relocated back to their hometown and for no reason given was denied. One of the family members has used the facility before. God Sees All!
