My husband was admitted to Englewood in Monroeville for rehab to strengthen his upper body. Patient care was well below normal. My spouse was left in soiled underwear for over an hour on at least two occasions. CNA was administering meds to my spouse which is not in their scope of practice. A medication was given to him that caused him to not be able to wake up for a couple of days. He became dehydrated and very disoriented and his level of consciousness decreased. Finally after I continually questioned his care he was transported to the hospital and was dehydrated and found to have cdiff. His wedding band and bottom dentures were somehow lost in midst of all the chaos. Majority of the facilities faculty showed little to no concern for my spouse or the decline of his health while he was a patient there. There was not one person who took responsibility for his care. They just wanted to CYA and place blame everywhere except to the ones supposed to be caring for him while he was in there care.

Summary of Concerns During Mom’s Physical Therapy Stay (11/5/25 – 11/26/25)<br><br>My mother was admitted to the facility for 20 days of physical therapy beginning November 5, 2025. During her stay, we encountered several serious issues that compromised her care and safety:<br><br>1-Communication Failures Between Shifts<br>- On the first night, we informed staff that my mother had injured her right side, was unable to use her hands, and could not walk. The nurse aide assured us she would document this and ensure someone fed her. We were advised to go home and rest. Upon returning, we found that food had been given to her to feed herself, resulting in food scattered everywhere.<br>- Despite repeatedly informing staff that my mother was in severe pain in her right side, arm, and hands, each new nurse or aide attempted to take her blood pressure using the affected arm, showing a lack of communication during shift changes.<br><br>2- Unsanitary Conditions (Roaches)<br>- Multiple roaches were discovered in my mother’s suite. After returning from grocery shopping, we found the countertop covered in roaches. Staff acknowledged this was a recurring issue but said no one was available to address it that evening. We were forced to move her to another room.<br><br>3- Medication Administration Errors<br>- Staff instructed that Norco be discontinued, and a nurse confirmed it was stopped. However, during the next shift, another nurse attempted to administer the drug. This lack of communication between shifts created a dangerous situation.<br><br>4- Equipment Mismanagement and Inconsistent Information<br><br>- Over the weekend, I requested that staff use a lift to place my mother in her recliner so she could continue exercises as recommended by her physical therapist. A nurse claimed the lift charger was damaged by a patient and that no other charger or lift was available. Later, another staff member said the charger had simply been misplaced. Eventually, an administrator informed us that multiple lifts and chargers were available in the facility. My mother was finally transferred to the recliner, but the conflicting information and delays highlighted poor management.<br>-Earlier in the week, we had already been told that weekend staff were “lazy,” which seemed consistent with the lack of effort and misinformation we experienced.<br><br>The best part of this facility is the physical therapist. I give them 5 stars.
