We will forever be grateful to this rehabilitation center and the incredible staff who work there. My mom spent a full year, and when she arrived she could not walk and had a 22 inch open wound. Today thanks to the dedicated staff she was able to go home. A very special thank you to the PT dept. Specially Steve,Liz Maria and Rene. Your hard work and dedication changed her life. Kenya the best and most amazing wound care nurse. Who week by week cleaned,and help heal. Now still open but about 4 inches and looks beautiful. We also want to recognize Rosa,Sue Maribel,Arnie,Candice,Patrick,Louie,Sheena, Kris,Cora,Jackie Crystal,Jen and so many more that supported us along the way. There are honestly too many names but please know you are appreciated. Thank you to the best medical Provider Richard! The best, no words to thank him for his care. And always available 24-7. Thank you to Social service Dominique and Alyssa. Our caring nutritionist Brittney,and our weekend greeter Lizzy. Aracely fir always setting up transportation. I know I am missing names, but know this year could mot have happened without each and every one of you. Keep going! Keep caring! You didn't just provide care you became family. This rehab is awful service and most staff is lazy. Both my parents are currently there, I have question's that no one seems to be able to answer or they give you the run around Doctor's are supposedly there at 7:00am but family cant arrive until 8:00am so how can I discuss a concern to the Dr if I cant show up until after the Dr. leaves. I need help and answer please and thank you.

I don’t have enough words to describe how great this place is. Even though I was going through a rough situation, I felt loved here and taken care of. I came without been able to walk use the restroom or even get up the bed. My rehabilitation team was amazing, Miss Anita, Yelena, Neal, Maria everyone on the team was such of foundation for me to be able to do what I’m doing rn. My Nurses were always on point with my medication Miss Shelly, Ruby, Victoria, Angel, always fun to talk to and spend my time. My CNA would never let me down in anything Joseph, Miss Angela, Evie, Catalina, Precious, Francisco. The whole team was great and amazing. Peoria Post Acute is a place where you can rest your shoulder on. I leave but I made a lot of friends with staff or even patients. I would never forget my conversation with the patients.

My experience with this facility was extremely disappointing and uncomfortable. The staff was consistently rude and unprofessional toward both my spouse and me. I was often completely ignored when in the room, and staff made rude remarks about me and spoke negatively about both my spouse and other patients. What was most disturbing was hearing staff mock patients who were crying out for help, including making degrading comments about them within earshot of us. This shows a serious lack of compassion and professionalism. Staff also had no sense of personal boundaries — at one point, staff sat on the bed with my spouse and me, which was inappropriate and made us very uncomfortable. Requests for help were routinely ignored, and when I tried to ask questions, staff would walk away instead of responding. I was also told I could not be included in conversations about my spouse’s care, despite having the legal right to be involved and my spouse clearly consenting while sitting right next to me. This was both frustrating and unacceptable. Overall, this facility failed to provide basic respect, communication, and dignity to patients and their families. I would not recommend this place to anyone seeking compassionate or professional care.
