I would have given 2 stars had they not lost personal items and mixed up patient info.

I am in absolute shock over the quality of care here. Unbelievably poor. If a higher up sees this review he should contact me immediately.
The nurse poked a hole in my vancomyocin antibiotic bag and didn't tell me and just put a black paperclip on it to stop the leak. She was going to put that in my picc line. When I told her that it wasn't sterile anymore and I have a bad infection in my new knee replacement she went out and got an emergency supply that was half the dose.
I have never seen such shoddy care...EVER.

I would not recommend this facility. The rooms and furniture are in poor condition and the floors are filthy. Overflowing bug traps and spiderwebs can be found behind the furniture. My father was taken here after a hospital stay and we were never provided any paperwork or a tour of the Facility. The physical therapists insisted on getting my father try to sit and walk when he clearly could not. After a week my father was released on hospice and sadly he died a few days later.

The only reason we ended up here is because the hospital could no longer keep him and we were forced to make a decision from a list handed to us in the hospital. This location happened to be the closest. The guilt I feel for the week he stayed here is unbearable.

I run a mobile xray business in Yavapai County. I work in every facility for elderly care every day, and this facility in particular is one of the cleanest and most effectively ran that I know of. Definitely top three out of the dozens I service. Clean, professional, and has a cozy feel compared to many of the other options available. I would absolutely have my own mom live here, given the need.

I was at Good Samaratin PV. I had a mild stroke, was not able to walk well when I arrived in a wheelchair. I was there for 3 weeks. From the time I was picked up at the hospital by Jeff to arriving speaking with Amber to meeting my CNAs and Rehab. I felt so comfortable and new this was the place I belonged. After a few days meeting my Nurses and meeting with all PT, OT and ST was amazing they worked so hard to get me stronger to be able to walk out of there. Plus all of the other staff and friends I met. I will never ever forget this wonderful experience that I had. Thank you to all of you at Good Sam.

About this data
How to read this page
Star ratings vs. Care Quality Score: The Google star rating reflects visitor and family opinions collected from public reviews. The Care Quality Score is an independent metric derived entirely from federal inspection data — the two measure different things, and comparing them side-by-side gives a fuller picture.
Where this data comes from
All clinical data on this page originates from the Centers for Medicare & Medicaid Services (CMS), the federal agency that certifies and inspects every Medicare- and Medicaid-participating nursing home in the United States. Reviews are imported verbatim from public Google business listings.
