Couldn't get my mom out of there fast enough. Social Services were especially rude and unhelpful.

I have a very dear friend that was sent here. She is not able to help herself and was sent there for rehab and to help her regrin her strength, so it's easier for her at home. This is unacceptable. I do not understand how these places are allowed to neglect their patients or clients they are sent there.

Do not send your loved ones here. This facility has some serious safety and care concerns that need to be addressed. My grandmother was admitted following a stroke and received dangerously inadequate care during her week-long stay. I'm sharing this detailed account so other families can make informed decisions. Patient Safety Issues - Dietary restrictions consistently ignored despite documented medical needs - Incorrect medication administered for her condition - Discharged with ANOTHER PATIENT'S MEDICATION (critical safety violation) - Found alone in the bathroom after 15+ minutes of calling for help Neglect and Poor Care Standards - Received only ONE shower during entire week-long stay (and only after I personally requested it after multiple requests) - Physical and occupational therapy schedule provided just once during stay - Facility had a persistent foul odor throughout Communication Failures - Zero callbacks despite multiple voicemails to staff - No updates provided to family about her condition or care plan - Repeatedly given false information about her transfer status - Family not notified when she was finally transferred Transfer Coordination - I had to physically come to the facility to initiate her transfer - Staff did not coordinate with receiving facility - I had to do this myself - Lied to repeatedly about transfer timeline The nursing staff appeared skilled but severely overworked. The real problem is management and operations, which seem both incompetent and borderline negligent. The facility is outdated and in poor condition. My grandmother deserved dignity and proper medical care. This facility failed on every level.

I would not recommend this place for anyone. My mom was in there with an amputated leg. I don't think they did enough for her to gain any strength. They wouldn't even let her utilize the toilet. They took her television away as she needed it to be up louder than normal due to hearing loss. Her telephone in her room did not work. She stated that the food was terrible. I myself would go and get food for her to eat. My grandmother was there many years ago with the same situation and they took very good care of her. I wish I had chose another facility for my mom.

My mother was there for therapy and she was on one side of the building and she was moved to the other side on her shower day, and when I got there I asked if she was going to get her shower they said NO. If I want her to have a shower then I can do it myself. I did. This is the worst so called rehab in Tucson. The staff walks the halls talking about the patients and laughing at them. So rude.95% of the staff shouldn’t even be there.

About this data
How to read this page
Star ratings vs. Care Quality Score: The Google star rating reflects visitor and family opinions collected from public reviews. The Care Quality Score is an independent metric derived entirely from federal inspection data — the two measure different things, and comparing them side-by-side gives a fuller picture.
Where this data comes from
All clinical data on this page originates from the Centers for Medicare & Medicaid Services (CMS), the federal agency that certifies and inspects every Medicare- and Medicaid-participating nursing home in the United States. Reviews are imported verbatim from public Google business listings.
