I am a hospice nurse and I frequently visit Santa Rosa to see my patients. The staff here are so nice and I have never had an issue communicating with the nurses. Everyone works well together and you can tell the staff genuinely care about the residents.

Santa Rosa is under new management, and the care provided is truly exceptional. From the moment we arrived, we felt genuine compassion and kindness from every single staff member. The clinical and therapy care went above and beyond, and it was clear that our loved one was treated with dignity and respect. The staff took the time to listen, answer questions, and support our family every step of the way. During such a difficult time, their care and understanding meant more than words can express. We are deeply grateful!

This place has deteriorated significantly since the new management company took over a couple of months ago. They have cut corners on the quality of care by reducing the staff-to-patient ratio, which has caused a significant decline in the care residents receive. My relative is completely dependent on the staff for all essential care, including feeding, hydration, shaving, bathing, changing diapers, and dressing, due to his disability.<br><br>Here are specific incidents illustrating the unacceptable standard of care:<br><br>Dehydration and Neglect:<br>I visited my relative after work around 5:15 p.m., and he was laying in his bed looking miserable.<br>He mumbled that he was thirsty and had been yelling for assistance, but the staff was too busy running around to assist him with fluids.<br><br>Prolonged Soiling and Inadequate Staffing:<br>On another visit around 5 p.m., I asked a CNA when my relative had been moved from his bed to his wheelchair. The CNA stated he had been up since the morning and not placed back in bed. My relative is a hemiplegic (paralyzed on one side) and wears diapers because he cannot use the bathroom independently. This meant he had been sitting in a soiled diaper all day.<br><br>Dismissive Response to Concern:<br>"I informed the CNA that my brother's clothes were wet due to the soiled diaper. The CNA apologized, explaining that due to understaffing (specifically, only two staff members were available on the floor), they did not have the capacity to change him at that time. My relative was not changed until after the dinner service had concluded for all unit residents and were being transferred to their beds/rooms."<br><br>These incidents are just a few examples of the many that have occurred under the new management. This level of neglect is unacceptable.<br><br>Fortunately, we were able to move my relative to another facility, ensuring the new location was not affiliated with the same management organization."

About this data
How to read this page
Star ratings vs. Care Quality Score: The Google star rating reflects visitor and family opinions collected from public reviews. The Care Quality Score is an independent metric derived entirely from federal inspection data — the two measure different things, and comparing them side-by-side gives a fuller picture.
Where this data comes from
All clinical data on this page originates from the Centers for Medicare & Medicaid Services (CMS), the federal agency that certifies and inspects every Medicare- and Medicaid-participating nursing home in the United States. Reviews are imported verbatim from public Google business listings.
