Do NOT take your loved one here. This place was horrible. My dad went downhill after being here. The day we admitted him, no one took care of him until the next day. They didn't even provide food or have a room ready for him. One time he called for help, and the nurse yelled at him saying, "You are not the only patient, I have other patients here too.". They need to shut this place down. My father was so traumatized from how they treated him there, he had nightmares and was mentally scarred. It breaks my heart to see how healthcare providers can treat people like this.

Find another place for your loved one as this place has gone downhill. This was the 3rd time my dad has been here. No problems the past two times, but boy lots of problems this last time. He was admitted in May 2025. Severe lack of communication with the lead charge nurse and a joke of a social worker. The administrator agrees that the head nurse and social worker cover each other. I stayed with my dad about 8 hours a day in his room, helped him eat and go to the bathroom. I feel my dad was given incorrect medication which caused him to just be knocked out daily and hallucinating. After only 6 days of being there we got notice that the insurance wanted to kick him out. My 90 year old dad could not get out of bed or move, yet PP did nothing nor would give me what they submitted to the insurance, so our appeal for him to stay was denied. The social worker must get kick backs from referring you to a company that finds private care. I told both the charge nurse and social worker that their communication skills suck! My dad laid around for 8 hours waiting for them to come to us on how they were going to transport my dad to a private facility, then my dad laid in the guerny for over a hour waiting for them to get their act together to discharge him. I asked for a copy of all my dad's reports from them, which they did not provide. No doctor ever saw my dad or evaluated him. My dad was experiencing severe pain from a new diagnosis of Spinal Stenosis...which they stated that Pomoma Valley Hospital was not responding to their request for the resultsof his CT scan, so they had no idea of what type of therapy to provide my dad. If it wasn't for my dads primary Dr following up with me we probably would have never got his diagnosis. I got it and took it to the charge nurse. All she wants to do is run from you every time you have a question. It was a horrible experience for my dad, the food was awful and the lack of communication was astounding. There were several CNA's that were very good and helpful to my dad, and several who were terrible. A real shame as we requested for my dad to be transferred here from the hospital. The owners and administrators need to do a overhaul. Claremont is known for seniors and Pilgram Place should have a excellent reputation...not anymore.

If there was a negative rating to report on this place I would give it a HUGE negative. My mom was sent to Pilgrims Place Health Services following a recent hospitalization. This place is an absolute joke. Within the first week a CNA let go of my mom and she fell and hit her leg. It was not documented initially and no one called me. It wasn't until my mom kept complaining that her leg was swollen and she couldn't place weight on her leg that they took an xray of it. Thankfully, nothing was broken just severely swollen . To top it off, during her stay, she was NEVER seen by a doctor, yet allegedly doctors signed off on paperwork sent to medicare. To make matters worse, she was almost given the wrong medication during her stay. Luckily my mom knew her medication and refused to take those that the administrating CNA OR LVN was trying to give her. Her insurance wanted her to be released and sadly this facility did NOT accurately report my mom's current findings that I lost her appeal for more time. Their social worker is a complete joke. I don't know why she's there if she's not going to help the consumer or families with knowledge of what to do or how to apply for medi-cal. They do NOTHING at this facility to help families navigate the system. I had no idea what i was doing. I spoke with several individuals in the medical field as well as DPSS trying to get my mom emergency medi-cal and they couldn't believe the social worker there did nothing to help me.

Well, my mom was released/discharged on 12/19, after my attempts to keep her there under her insurance coverage, and my worst fears came to fruition. We got her home. She had NO strength in her legs, and her legs gave out and she fell face forward. There was nothing we could do to prevent the fall. I had to call 911. I was back at the hospital less than 3.5 hours after her release/discharge.

Think twice about this facility! Oh by the way they will try to get you to sign a paper that charges you $500-600 a day to reserve a room for your loved one IF they go back to the hospital. I was told this may be illegal. Like a dummy I signed it not knowing better.

Look at the prior Yelp reviews. I dealt with some of the same people referenced. I intent to advise medicare as to their lying practices. The system is flawed for the elderly.

When our loved ones are older, we need to be their voice and you beat believe, I'm being my mom's voice!

My mom has bites on her

If we wanred to private pay

Don’t go unless you hate your loved ones. Neglected my grandmother, Atika. Neglected our many many pleads for a doctor. Now she’s not here. All useless nurses who shouldn’t be in this profession.

About this data
How to read this page
Star ratings vs. Care Quality Score: The Google star rating reflects visitor and family opinions collected from public reviews. The Care Quality Score is an independent metric derived entirely from federal inspection data — the two measure different things, and comparing them side-by-side gives a fuller picture.
Where this data comes from
All clinical data on this page originates from the Centers for Medicare & Medicaid Services (CMS), the federal agency that certifies and inspects every Medicare- and Medicaid-participating nursing home in the United States. Reviews are imported verbatim from public Google business listings.
