My mom was in need of nursing assistance until we were able to get ihss for her at home. Not sure why this place accepted her when it was mentioned to them that she has dementia and she’s wonders. They assured us there would be a wonder guard on her. The first day we went to visit nothing, the nurses in the front didn’t really know about her situation. My dad went to visit Friday and mentioned when the nurses were trying to put an iv they were hurting her and she started crying, he asked them to stop I went by later that day had dinner with her and when walking her to the room there was a bloody towel on her bed! I asked the nurses what was going on and they tried pinning it in her roommate and changed her bed sheets right away. I had to leave but she seemed ok. The next day my dad went to see her in the evening and she was not there!!! He asked where she was and they said they sent her to the hospital in the morning!!!! NO ONE called us. I called to complain turns out the night after I left my mom went missing for 45 MINUTES!!!! They only knew because her roommate let the nurses know she wasn’t in the room. They never called me to notify me!!!!!!!!! I called to speak with a manager and they told me there’s no managers on the weekends. Later I get a call from a manager with an attitude on defense mode with an accent telling me they were sorry etc I get a call from the hospital telling me the facility did not want to take her back (thank god)<br>If you have a family member with dementia DO NOT SEND THEM HERE!!! They seem to take only the ones that are in a wheelchair because it’s easier to ignore them when they’re being aggressive or can’t speak up for themselves

This is the worst rehab I have ever seen! I could not believe how uncaring most of the staff is. There were only a couple of nice and hardworking employees. No Dr on site. And if you try to find a nurse seems impossible. We took my friend there and she almost died. They let her to scream all night “help me”and she did not eat for days. The food is so disgusting and greasy. I would not feed that food too my worst enemy. I can tolerate a lot of rudeness but when it comes to elderly nope not doing it. That’s why I’m writing this. Don’t go there if your loved one can’t speak up for themselves. In fact please don’t go there at all! We almost lost our loved one. I told staff that they needed to call 911 because she needed help and was sick. The nurse said that her and the other nurse did not feel it was urgent but that a non emergency transport was coming. When we got to hospital Fontana Kaiser. They quickly took her in and said she was very sick and needed nutrients and had no oxygen. Dr said it was good we came in. Now she is safely in hospital trying to recover. But 4 days without no food and no sleep because of how was treated at Citrus nursing in Fontana will forever be a horrific memory for me. Oh and the nurse laughed at me when I asked if patient got sleep. And said no. I did not know what the heck was so funny! That urked me! Dirty place. And By the way patient is in good hands now best staff over here in Ontario Kaiser! She does not scream help me. Food is great here! I have more to say but will not. I don’t want to ever talk about this situation again. And No Fontana nursing center I do not want to talk to you. I’m done with that place.<br><br>Update One thing I forgot to note after this she was transferred to hospital and died less than a week later. she died on 4 th of July. Maybe next time they will call 911 on first night of no sleep and screaming!

My mom has been their quiet a while and she loves that place even tough I don’t live close to her she always tells me she loves the cna Destiny she always makes very good comments about her that she does a very good job with helping her with all her essentials and is a very caring cna to her and she adores her personality and does gets upset when Destiny isn’t working.

My mom has been in this facility for many years due to the amount of care needed to manage her ilnesses. I have seen both good employees and bad employees come and go. I don't live in the area so I depend on the staff to make sure my mom's mental and physical health are being taken care of. The activity department and majority of the CNA's are very caring. Especially the ones who have been there for many years and have personally cared for my mom. I would personally like to name a few individuals that have stood out to me who deserve some extra attention. Zulema Palencia has got to be one of the most caring people that I have ever met. She will go and see my mom at times when she is off or during her breaks to reassure me that my mom is doing good. She will also FaceTime me so I can see her. She should be given a pay increase for going above and beyond. Jessica Cintron is a nurse that also goes above and beyond. She reassures me all the time about my mom's condition. I am going to name other people and I hope I don't leave anyone out that is deserving. David Palencia, Mercy, Alice, Gabby, Reen, Pierson, Rosita, Rosa, Ines, Brittany, Kayla, Kyla, Miranda, Crystal and Jennifer. When I've had problems, The Administrator, Walter did his best to try to help me solve them. Thank you to all of the people who have made a sad situation more tolerable. The facility does the best they can with the shortage of staff. I love you guys! ❤️😊

Physical therapy mike and Linda are absolutely the best social workers are excellent with the exception of one name Linda who is rude and will destroy you outpatient paperwork the nurse are fun and pleasant the two top dogs Walter and yenni and excellent listener and make sure your very well taken care...I had a excellent stay would recommend this facility to anyone best so so far
