We brought our loved one here following a hospitalization for skin breakdowns and reconditioning at another facility. The goal was to improve ability to reposition herself in bed and assist more with her own care, while also improving skin integrity and searching for a new “home”. She was there approximately 2 weeks. Carla, the main caregiver taking care of our loved one, was a force of nature; patient, skilled and had a truly magical way of charming and otherwise difficult patient. We are so grateful. Louis at reception is the perfect “face” of the facility. Flawlessly warm, efficient and knowledgeable, he keeps the hub of activity at the front entrance flowing smoothly, even while chatting away with a patient (who was certain she had lost her way) as if she were an old friend! Suemy Madrid and Tamara Rodil kept me informed regarding care and facilitated assessments, placement in a new facility, communication across multiple agencies and personal needs - professional prowess which was desperately needed. The staff- as a whole - were amazing. Thank you!

It's been almost a year since my dad died here and all I got was a call that he didn't have a pulse. When I asked the nurse why this happened she said it could have been because his blood sugar was low and I gave him sugar (as she seemed SUPER nervous on the phone) This was never documented. I was told by her that a CNA went in there and asked if he needed anything and he said no but the CNA had told the nurse that he seemed disoriented AND NOT MAKING SENSE. (This wasn't documented either). Then I was told that the nurse didn't come in there for another hour after the CNA reported him disoriented. Although he wasn't a diabetic.. why did she verbally tell me she checked his blood sugar and gave him sugar. Knowingly you can't have sugar with Kidney failure and heart failure. This is so sad because it's all he said she said stuff. After his death I tried to call the facility multiple times to get ahold of the nurse who cared for my father who died on 2/8/25 and they would put me on hold for her tell me she's there and end the call or give me to another nurse. He had kidney failure, pulmonary hypertension, PAD, heart failures and he had surgery in his legs for the blood flow. He was there because of the surgery to try and walk again. Every time I talked to him on the phone there he sounded like he was in immense pain. The last time I talked to him on the phone he had told me that there were things going on at the facility and he couldn't talk about it at the moment and wanted me to get down there. He was very nervous. Before that I couldn't get ahold of him for a week while he was there. Because of personal medical reasons I couldn't make it down there and told him I'd be there in a few days. Little did I know that was the last time I would ever hear from him. He was dying and no one even considered that he was even after the CNA told the nurse that he was disoriented and not making sense. I was told a doctor only goes in there once a week as well. After he passed I looked at his phone and his messages to others didn't make sense, and he tried to reach out to someone while he was in the facilities care to try and tell them he thinks he may of had a stroke in there. If he did, it went unnoticed and he died. This is all so sad and the burden that I carry until today is that these facilities don't care about the elderly with multiple diagnosis. I thought he would be in good hands and that was my fault for thinking that. I thought it would be a good place because the religion aspect and my father lived far, but I'd have to say you'd be better off getting your care from home health care at home/hospice. Maybe if you're in there to regain strength from an accident but otherwise I would not send your elderly parent there if they have a chronic disease. Please don't do it.. my dad may have not been easy to deal with but they didn't catch the signs of death and in my opinion that means that these poor people aren't being evaluated or watched enough. God knows who this person is and what actually happened in that facility the night my dad left this earth. I'll miss him forever and will never have my loved ones ever go to a facility like this again. The pain is deep and it's just not worth it. RIP dad...

I was transferred to St. CATHERINES, after a couple major surgeries to recover. I had a major wound that needed a Wound care specialist. I was lucky St Catherine's had an excellent wound care team. I was basically slit open from front to back with deep open wounds. The wound was big. But the wound care team knew what they were doing. After one month I was able to go home w/ in home care. St Catherine's team helped me out. I would highly recommend St Catherine's. For long or short term stays. Special thanks to Joe & Mark wound Dr. (The wound care team) And everyone who made my stay good. The nursing staff & CNAs the office staff. And Francis with an I on activities. THANK YOU ALL ***** 5 stars

About this data
How to read this page
Star ratings vs. Care Quality Score: The Google star rating reflects visitor and family opinions collected from public reviews. The Care Quality Score is an independent metric derived entirely from federal inspection data — the two measure different things, and comparing them side-by-side gives a fuller picture.
Where this data comes from
All clinical data on this page originates from the Centers for Medicare & Medicaid Services (CMS), the federal agency that certifies and inspects every Medicare- and Medicaid-participating nursing home in the United States. Reviews are imported verbatim from public Google business listings.
