A huge thank you to South Marin Health and Wellness for taking such excellent care of my husband. They provided exceptional care and were readily available to help with any and all of his needs. Shae in admissions, Kelly the social worker, Ciara in nursing, and Gabriel his caregiver, just to name a few, were always available to family members, and addressed and solved all concerns that arose. The OT and PT staff are way above par. I felt very fortunate to know he was being so well taken care of.

My Mother has (unfortunately) needed rehab stays several times in the last few years at multiple facilities. South Marin is by far the best. Their PT (what she tends to need most) is terrific - the PT therapists seem to experience little turnover as the same individuals have provided PT for my mom over several stays. Despite being very busy, the social workers are also very helpful. And Dr. Susan Nguyen (the hospitalist there) is wonderful - caring, smart, and on it. Rehab isn't fun, the facilities are all really busy and frustrations are inevitable. But we will keep requesting South Marin when my Mom needs it b/c it's the best around.

My mom has been here for over a year. Her dementia is getting worse and they are always here to support her. The staff is excellent, food is awesome, and they go out of their way to make my mom feel comfortable in her new home. Oh, and all of the residents are super nice as well!

South Marin is conveniently tucked away next to the doctor’s offices on South Eliseo and walking distance to Marin Health Medical Hospital.

The location is handy, but it is the kind, compassionate, knowledgeable care provided by the nurses and staff that makes South Marin a good place to stay for rehabilitation.

The gym is open everyday, and the Physical Therapists are hard working, conscientious and strive to help the patients get back on their feet.

The food is good with a wide variety available. Everyday there are different activities including the ever popular Karaoke and many different movies. There is a small patio outside and a very pleasant interior atrium.

Dr Wynn, the attending physician at Marin General, recommended South Marin and it turned out to be an excellent choice.

- Michael Brink-
April 2025- June 2025

About this data
How to read this page
Star ratings vs. Care Quality Score: The Google star rating reflects visitor and family opinions collected from public reviews. The Care Quality Score is an independent metric derived entirely from federal inspection data — the two measure different things, and comparing them side-by-side gives a fuller picture.
Where this data comes from
All clinical data on this page originates from the Centers for Medicare & Medicaid Services (CMS), the federal agency that certifies and inspects every Medicare- and Medicaid-participating nursing home in the United States. Reviews are imported verbatim from public Google business listings.
