I am writing this review from my wife's Google account. My mother was admitted to the Trabuco Post Acute property from the hospital for occupational, physical, and speech therapy after a stroke. I noticed that the facility has plenty of healthcare providers. They all seem be very busy with the patients, from physical therapists, nurses, case managers, social workers, admissions, business personnel... The healthcare staff seems very friendly and the culture at this facility is upbeat and positive. Whoever is in charge of running the facility or in charge of hiring the staff has done an awesome job getting the right people for this type of healthcare. This is a challenging work enviroment and Trabuco staff has been great. My mother has made a lot of progress in a very short time. She is improving daily and my siblings and extended family who have visited mom have commented how nice the facility is and how great the staff has been. We have been overly satisfied with my mother's care and progress and I highly recommend Trabuco Post Acute Skilled Nursing facility to anyone who wants their loved one to improve and thrive in a caring enviroment. Lately, I’ve been visiting my mother here more often, and every single time—from the moment I walk in the entrance to the very end of the building where she stays—I’m met with nothing but kindness and professionalism. Everyone I interact with is incredibly warm, respectful, and attentive. Honestly, I’m amazed. This place truly feels out of this world. I want to sincerely thank the management and every staff member for the care, compassion, and dedication they show—not only to my mother, but to all the elderly residents who need support and care. Your hard work does not go unnoticed. Five stars simply aren’t enough. This place deserves a million stars.

Trabuco Hills Post-Acute's care was a nightmare. The room was dirty, the shared bathroom floor had holes in it. The food was unappetizing and usually lukewarm. Worse of all, the nursing assistant's are not training or monitored. My mother got an E. Coli UTI that took her down for a couple of weeks and then C. Diff for another week after that. E. Coli in the urinary track is caused by stool being transferred to the urinary area during care. C. Diff is another infection caused by stool. My mother also had bruises that no one was aware of or knew how they happened. Lastly, I hid $40 in a glasses case in her drawer so she could get her haircut. I found that was missing when she was discharged. I filed a grievance and had a meeting with staff but the damage is done. My mother will never be the same.

My husband has been staying here, Trabuco Hills Post-Acute Rehab Nursing Care, since 10/5/2025 after being discharged from hospital. As a patient's wife, I sincerely appreciate all managing teams and staff members; not only directors in each dpts but also nurses, pcps, ot, pt, and all staffs are very kind, professional, well trained, and all the time to be ready to help a patient. Forthemore, the cleaning teams are very diligent, so facilities are clean. And its location is not near major fwy, so air quality and environment are much better than the others. So, I would like to leave my review of gratitude to all the members who are doing their best for the patients.
