This place has one of the worst book keeping practices I have ever experienced. Both of my parents were admitted to the Carlotta in Jan ‘25 after hospital stays. Before being admitted I was forced to prepay for 2 full weeks of service to the cost of ~$6,000 (they both have Medicare advantage plans but they said the difference would be refunded to me once claims were submitted which seemed off to me from get go). My mom was released in less than a week. It took 10 months to get any refund for her treatment because their claim kept getting rejected which means they weren’t coding things properly. My father was released shortly after. He passed away in June, it is now December, nearly one year later and they are now calling my mother and myself for an outstanding $500 balance for my father’s stay that they said has only just now been discovered. My father has been in two other similar facilities in the area before passing and I’ve never experienced such egregious accounting and billing practices elsewhere. Today I got a call from their business manager Sarah telling me she is moving to legal action if it isn’t resolved (which isn’t even my responsibility to pay). I’m the son of the patient, my mom is in her late 70’s this sort of behavior is just egregious to be calling family members of a deceased patient to pay a $500 bill they only just discovered a year later after treatment.. Avoid this place, terrible.

I place no faith in the CMS rating system. Data can be manipulated. For instance, my father did not pass away at the facility, but they were directly implicit in his demise. In fact, after three trips to the hospital from the facility, we chose not to return him to The Springs Healthcare Center at Carlotta, therefore his death was not reflected in their numbers.<br><br>September 2025 UPDATE***************** I have just been informed that The Springs Healthcare Center at the Carlotta is in process of settling a Class Action Suit for $12,000,000.00 for failure to meet the California Nurse Staffing requirements. Had this information been public I would not have placed my father at this facility. This further substantiates my belief that they were complicit in my fathers demise.*******************<br><br>My 91 year old father did not receive the care he needed to recover from a hip fracture.<br>I have learned that the management of The Springs at Carlotta has changed hands since their 5 star Medicare rating. They would not receive this rating today.<br>There are too many areas of concern to address here but I need to provided at least the following as a warning to whomever is interested.<br>Orders from the hospital stated that my father needed to be up and moving 3-4 times per day. He had been up and walking with a walker in the hospital. The Carlotta staff told me he was a fall risk, and the orders they had on file said he should not put weight on his injured leg. This was nowhere in the hospital orders.<br>When I pointed out what the Surgeons orders were, they stated they do not have the staffing to assist a patient getting up 3-4 times per day. How do you qualify as a rehabilitation facility when you don't have the staffing to support a successful rehabilitation?<br>They provide a list of healthcare assistants you can contact and hire/pay for on your own to provide care. This isn't revealed on their website.<br>They do not have a doctor on site and when asked cannot tell you when exactly the doctor will be on site. Critical communications regarding my dads health were either delayed or missed.<br>Several times medications were administered inaccurately.<br>There are several stellar staff people but overall they are understaffed and not providing adequate care to all patients. There continues to be alot of turnover in staff.<br>My father ended up with a bed sore, a UTI, ESBL and in quarantine for this, and blood clots due to lack of movement. It was a traumatic experience at The Springs at Carlotta.

I am extremely happy with Carlotta all their staff is caring and always willing to help. This facility is an amazing and the dining experience is amazing as well. The kitchen makes such good food— Does NOT compare to hospital foods! It’s always a heartwarming to see the way staff cares for patients.

My friend was admitted for rehab s/p a hospital stay for cardiac issues. Several weeks into his rehab stay, he was injured during PT while at The Springs. He sustained a torn meniscus and effusion (swelling and pain). The physician would not return my calls and a formal diagnosis took almost 2 months. There was no treatment other than occasional ice. Because he could only complete PT on one leg, Medicare stated that he was no longer meeting medical criteria. Since he could no longer use his now injured leg, he could not progress with PT!<br>The Springs then began charging him $450/day. I was able to transfer him to an assisted living as he could not return to baseline. I attempted to speak with the administration and they were not aware of his injury and extremely unhelpful. The was absolutely no concern for my friend. The social services and administration never once reached out to friends of family. They were content with my friend not understanding the system.<br><br>He was discharged in worse physical condition than when he was admitted. He now has an inoperable knee injury.<br>This is unethical and criminal elder abuse.<br>I am a nurse and patient advocate. I involved the CA Ombudsman last month and they have opened an investigation.
