It takes Forever to respond to your call light there is no excuse for it. It’s highly Disappointing all they do is Yap and screw around the nurses and CNA’s are not professional. The state needs to come in and review this place.

Dustin, Admissions Director, should be elevated to Executive Director. He is the only one who communicated with us throughout my husband's 1 week stay, and beyond, for transition issues. Dustin has the level-headed, common-sense, hands-on approach to care Premier really needs. He was the only one available to even try to reach the doctor, nurses, PT, executive director. You cannot get any care here despite how hard you try and working with the Director of PT, DON and facility director, it's IMPOSSIBLE. My husband was prescribed a wrong medication for his condition. The doctor did not catch it and didn't contact me for a week, despite my husband requiring a family POA. Someone on staff caught it after 7 days of my husband's worsening condition, most likely due to contraindicated meds (benzodiazapines for dementia patient, a BIG NO-NO). These suddenly worsened my husband's condition (constant falling, combative, aggressive, delirium), which both the Eisenhower hospital and Premier staff attributed to his "declining illness". Neither listened to me that immediately prior he was fully ambulatory, pleasant demeanor and largely oriented. Many thanks to the unknown staff member at Premier who finally caught this error (though I wish it was found on day 1, not day 7) And many thanks to Dustin for his even-keeled demeanor and common sense.

I came to see my mom and we went to the activity room and i had to just stand there while they played bingo. The staff did not offer a chair or anything. They are not very accomidating to the guests that visit.

The dude who works in admissions was rude when inquiring about care for my father. Did not ask for his opinion on the type of care he needs and he felt obligated to state so. Also he had an attitude to begin with when asking for admissions. Oh I’m sorry we called before you close. SORRY YOU HAVE TO WORK!!!

About this data
How to read this page
Star ratings vs. Care Quality Score: The Google star rating reflects visitor and family opinions collected from public reviews. The Care Quality Score is an independent metric derived entirely from federal inspection data — the two measure different things, and comparing them side-by-side gives a fuller picture.
Where this data comes from
All clinical data on this page originates from the Centers for Medicare & Medicaid Services (CMS), the federal agency that certifies and inspects every Medicare- and Medicaid-participating nursing home in the United States. Reviews are imported verbatim from public Google business listings.
What to do next
Data is a starting point, not a final answer. We encourage families to visit facilities in person, observe daily routines, and speak with staff and residents. See our guide: Questions to Ask When Visiting →
