Me and my Family have experienced the FINEST and most CARING of souls here at Sunnyvale Post-Acute Center! My Dad was here a year ago, and my Mum just recently. Both of them were brought back into action of every day life by the wonderful skilled nursing staff here! It was an absolute pleasure to have gotten to know the wonderful staff here - they are all very personable, caring and skilled. I always thought I was a ‘fairly’ good-person - But these guys and gals take the biscuit!! - I BOW DOWN to all of them - they work so very hard here to make a difference in people’s lives! I’ve seen the staff here SACRIFICE their own personal time - lunch times, breaks, days off, ‘after work’ plans - just so they can take care of their patient’s needs… (they don’t even let the patients/visitors know this due to your kind, humble and professional natures) - but when it comes to ‘human kindness’ - my RADAR goes off and I notice even more of their amazing kindness!! I’ve also seen them jump into action and support each other as a TEAM too! Loved seeing how well they all get along so famously - Excellent teamwork! The SINCERE and heart-felt COMPASSION that they all demonstrate via their ‘NATURAL way of being’ - through their ACTIONS, their WORDS, their PATIENCE, their SKILLS, their LOVE - is a lesson in ‘human-kindness’ to us all! Personally, they ALL taught me SO MUCH about how to ‘handle’ challenging situations (especially ones where I might sometimes fly-off the handle because I don’t know how to respond to awkward situations - due to my ego, patience & lack of ‘know how’) - They all taught me how to handle it with LOVE and KINDNESS just by their behaviors and interactions! I’m just ‘one’ example of the lives they’ve made BETTER and I THANK them for LEADING the way when it comes to human kindness! They rock at that! Being as wonderful as they are takes EFFORT, some of it is learnt through experience (the rest of it comes naturally to them) - Sometimes this effort they’ve accomplished ‘solo’, sometimes it might take a ‘village’, but at the end of the day, it takes ‘THEM’! Continue the GOODNESS like you ALL do!! I look up to the entire staff here at Sunnyvale Post-Acute Center (we ALL do - Patients, Family, Visitors and Strangers too). My husband has been here several weeks now and has been making progress toward regaining enough mobility to come home hopefully soon. The staff have been top notch in caring for him, arranging transportation to medical appointments and in every other way. And everyone has been super responsive to any concerns we expressed.

I am a patient here and everything went really well. I am getting discharge and it is hard to say bye to everyone. From the nurses to the therapy staff, everyone has been so helpful. The therapy team helped me get back on my feet and were very patient with me. The facility is very clean and odor free. I will miss the staff here.

My mother recently completed her stay at Sunnyvale Post Acute, and I cannot speak highly enough about the outstanding care she received from this incredible team. From the moment we arrived, Jessica in Admissions made what could have been an overwhelming process feel completely manageable. She explained every step of the intake process, helped us understand what to expect, and ensured all paperwork and assessments were handled smoothly. What truly stood out was how she was always on top of everything - proactively reaching out to check if there was anything she could do to help our family during the transition. Angelica, our Kaiser case manager, went above and beyond to ensure my mother's case was handled with utmost care and diligence. She expertly bridged insurance requirements with my mother's care needs, advocated with insurance, ensured coverage for necessary services, and helped us understand available benefits. Her dedication to getting every detail right was exceptional. Kaiser is lucky to have her. Renee, our RN, handled the crucial transition from hospital to facility with exceptional thoroughness and attention to detail. She ensured all the complicated medical information was transferred correctly and nothing was overlooked during this critical handoff. The daily nursing care was provided by a dedicated team of RNs who monitored medications carefully, kept our family informed about medical status and progress, and were always available to answer questions about the treatment plan. The clinical expertise of the entire nursing staff gave us tremendous peace of mind. The CNAs, including Alvin and Maheen, provided daily hands-on care with incredible patience and kindness. They assisted with personal care and mobility while treating my mother with dignity and respect every single day. Their warmth and caring nature made such an impact that my mother ended up considering them part of her family! I know there were other CNAs who cared for my mother during different shifts, and I'm grateful to each of them as well. The therapy team - Gargee, Kaavya, and Joe - designed individualized PT/OT programs that were key to my mother's recovery. They worked patiently to rebuild her strength and mobility, taught safe techniques for daily activities, and celebrated every milestone toward independence. What made Gargee and Kaavya especially wonderful was how they found creative ways to keep my mother occupied even during off days - suggesting biking exercises or activities she could do in bed. The special relationships my mother formed with them made therapy something she actually looked forward to. Dr. Rao and Dr. Lively provided excellent medical oversight throughout my mother's stay. Dr. Rao's thoroughness was remarkable - she maintained constant communication with our family through emails and phone calls, treating my mother as if she were her own family member. Dr. Lively brought such empathy to her care, spending generous amounts of time with each patient and approaching every interaction with utmost care and thoughtfulness. What truly set Dr. Lively apart was that she not only focused on making the patient better, but fully understood the caregivers' needs and addressed them with anticipation. Both doctors carefully monitored her condition, adjusted treatments as needed, and communicated clearly with our family about progress. Matthew, the social worker, was invaluable in discharge planning. He connected us with community resources and helped navigate the complex process of ensuring my mother had everything needed for a successful transition home. And Revathy at the front desk deserves special recognition for being such a warm, welcoming presence throughout our experience - her friendly face and efficient coordination made every visit feel more comfortable. If you're looking for skilled nursing care that combines clinical excellence with genuine compassion, I highly recommend Sunnyvale Post Acute. This team truly made all the difference in my mother's recovery journey.
