A huge thank you to North Park Post Acute for the care and help during my Uncle's stay. All of the staff was so friendly and warm. Carina from Social Services was incredible staying on top of applications and assisting us with his transition home and additional resources. I absolutely recommend this establishment as my personal experience has been great.

My father was at North Point Actue Care in Tracy for approximately 14 days. My father is 91 years old and although our family is small, when it comes to him, we are all there together. The staff there were ALL AMAZING!! EVERYONE was helpful with my father’s rehab as well as working with our family and the millions of questions we had. They made sure we were set with everything we needed when we came home!! We are grateful for the care my father received from North Point Acute Care highly recommend

I have been at this facility for over 2-weeks and I am so pleased and grateful for the care. All the staff, from management to the cleaning crew for all the help. The nurses go over and beyond. I couldn’t walk coming in, but now I am able to walk going home. Big Thanks to the Therapy team!

My husband's experience at North Park Post-Acute was nothing short of exceptional. He spent two weeks there after his right hip surgery, and we were both incredibly impressed by the entire team. They were not only professional and skilled but also genuinely warm and caring. It felt like they saw him as a person, not just a patient. The physical and occupational therapists were patient and encouraging, guiding him through his rehab. They understood his pain and frustrations and knew just when to push him and when to give him a break. Their expertise in helping him regain his strength and independence was crucial to his recovery. Beyond the medical care, everyone we met—from the nurses to the cleaning staff—was kind and friendly. There was a sense of community there, and you could tell they truly cared for the well-being of every patient. We are so grateful to have had access to such a high-quality facility during a difficult time. They made a challenging situation feel manageable, and we thank them for their dedication.

About this data
How to read this page
Star ratings vs. Care Quality Score: The Google star rating reflects visitor and family opinions collected from public reviews. The Care Quality Score is an independent metric derived entirely from federal inspection data — the two measure different things, and comparing them side-by-side gives a fuller picture.
Where this data comes from
All clinical data on this page originates from the Centers for Medicare & Medicaid Services (CMS), the federal agency that certifies and inspects every Medicare- and Medicaid-participating nursing home in the United States. Reviews are imported verbatim from public Google business listings.
