If you love your family member please do not allow Them to be sent here This has been the most nightmarish day at the end of my Mom’s 30 days away from Home. My mother was discharged today from Grand Valley healthcare on Sherman Way. After her 10 day hospital stay With sepsis, colitis and a UTI, Kaiser opted to send her for Physical and occupational therapy. She stayed there for 20 days. Through this time I was led to believe she was able to walk. I always came after her PT sessions which now I realize were basically to hold her up With a belt and have her take a Few steps. All this to satisfy them being paid by Medicare. She completed her 20 days and that was that. the transport brought her home and upstairs in a wheelchair and when I put her new walker in front of her to Stand up, she couldn’t. She fell back into the wheelchair. She is not Able to stand or move. I am beside myself in pure disgust from this so called skilled nursing facility. Her heels are peeling huge pieces of skin and they are causing her pain. No one there ever did anything to actually help her. Although there was a sign on her cabinet door that family would Be doing her laundry, their CN’s would take her soiled clothing and throw it in their laundry. I am missing so many pieces of her clothes that we spent money on to be altered since she’s so small. They also stole her slippers. All they do is what’s required by Medicare and nothing more. Patients are constantly screaming 24 hours a day and they leave the patients to marinate in their own filth until someone comes to Change them. Do not let your family stay here the food is vile.

As a family, we truly recommend Grand Valley. The staff was kind, patient, and genuinely cared about our loved one. They took the time to check in, explain things, and made sure our family member felt comfortable and safe. Knowing they were being looked after so well gave us real peace of mind during a difficult time.

About this data
How to read this page
Star ratings vs. Care Quality Score: The Google star rating reflects visitor and family opinions collected from public reviews. The Care Quality Score is an independent metric derived entirely from federal inspection data — the two measure different things, and comparing them side-by-side gives a fuller picture.
Where this data comes from
All clinical data on this page originates from the Centers for Medicare & Medicaid Services (CMS), the federal agency that certifies and inspects every Medicare- and Medicaid-participating nursing home in the United States. Reviews are imported verbatim from public Google business listings.
