My 94yo Aunt was admitted to Autumn Lake for rehab after being hospitalized for an infection. The rehabilitation department ( PT/OT/Speech ) was absolutely wonderful. Avy and team were always kind and professional - and understood my aunt's special needs due to her memory issues. Thanks also to Jose, Karen and Jenny for their hard-work and encouragement . My Aunt's condition improved with the team's help and she was able to return to an assisted-living memory care setting which was the goal.

K. Sule, Shanita and Barbara were very compassionate and attentive to my Mom’s needs during her 2 week rehab stay. The Physical Therapy team was great with her as well. April the nurse was amazing with wound care.

My mom is a resident here. We love the care she receives! The staff is excellent and professional. From the start when she was admitted we have been kept up to date on her care and involved in her care plan. CNA Naisha and CNA Theresa S. both have stood out on my recent visit. They work hard to be there for each resident and their needs. Amazing job, thank you!
