DO NOT SEND YOU LOVED ONE HERE!!!
I wanted to share my experience just in case you or someone you know might be considering it. Honestly, it's something I feel strongly about, and I wouldn't want anyone to go through a difficult experience there.

From what I've seen there are some pretty significant concerns right from the get-go. For instance, crucial initial health checks, like monitoring vital signs when a patient first arrives, seem to be frequently missed. Even more concerning, essential medications that should be given on the first day often aren't administered, leaving your loved one waiting 24 to 48 hours for their much-needed doses. This delay was incredibly stressful and detrimental, especially when your loved one is starting their recovery journey.
basic patient care and dignity can be overlooked. My loved one was sometimes left in uncomfortable and unsanitary conditions for extended periods. And trying to get information or an update? That's another challenge entirely. The phone lines are often impossible to get through, and on the rare occasion someone answers, it might even be another patient, which is pretty alarming.

Given these points, my honest advice would be to explore other options. Finding the right place for a loved one is such a critical decision, and I genuinely believe there are better centers out there that offer the attentive and professional service.

Apple Rehab is not a place where you should let a loved one stay especially someone who is injured and in a vulnerable position. My sister who has many health issues had broken her ankle and tibia and she spent three months here. The nursing staff is only there to administer meds the rest of the care is given by aids. They were constantly missing doses of my sisters medication and blaming it on the pharmacy. The aids would talk about other patients with my sister which was beyond inappropriate. I could write a book about how poorly run this place is but I wont waste my time. Just dont go there. Only plus was the professional PT and OT teams ( I think they may be an outside agency) Also Carolyn at the front desk is friendly and has no control over how this place operates.

My husband has been a patient for 2 years at Apple rehab he’s received the worst care any one can imagine. The DNS down to the nursing supervisor are all inadequate of handling any patient cares or concern. I have taken pictures and videos of them on their phone and their own private conversation instead of answering the call bell lights for the residence leading to multiple injuries from multiple residents falling out of bed and dying. Despite our initial hopes, the rehabilitation services have been consistently inadequate. Specific directives for his left arm, which suffered from a contracture, were disregarded, leading to painful pressure wounds. He also endured a severe fall, and subsequently, a medical boot was left on for two months, causing another significant pressure wound. Recent complaints of hand pain were dismissed with superficial "Band-Aid" solutions, resulting in untreated wounds.

Furthermore, his recent, unauthorized room change, conducted without consent or notice, caused immense distress, led to the loss of personal belongings, and physically isolated him from the nursing station. These systemic failures have directly resulted in multiple untreated pressure wounds.

I possess extensive documentation, including photographic evidence and testimonies from other individuals, detailing this neglect. I am actively pursuing all available legal avenues to address these severe breaches of care and ensure accountability. I strongly caution any family against placing their loved ones in this facility.

My Aunt went here for rehab after brain surgery. The staff was non existent. There were patients answering phones and the staff ignored my Aunts needs after a week she developed the flu which they acknowledged they had an outbreak of and they ignored her requests for help. Within the week she became septic went into cardiac arrest and died two days later at the hospital which advised our family should not have happened. She survived the surgery and they killed her a week later for utter neglect. Do not send your love one there!!!!!

About this data
How to read this page
Star ratings vs. Care Quality Score: The Google star rating reflects visitor and family opinions collected from public reviews. The Care Quality Score is an independent metric derived entirely from federal inspection data — the two measure different things, and comparing them side-by-side gives a fuller picture.
Where this data comes from
All clinical data on this page originates from the Centers for Medicare & Medicaid Services (CMS), the federal agency that certifies and inspects every Medicare- and Medicaid-participating nursing home in the United States. Reviews are imported verbatim from public Google business listings.
What to do next
Data is a starting point, not a final answer. We encourage families to visit facilities in person, observe daily routines, and speak with staff and residents. See our guide: Questions to Ask When Visiting →
