We were so pleased with the care, support and kindness at Autumn Lake! It had this homey feel that made you feel so comfortable! Someone came minutes after hitting the call button and that was a plus! We want to thank all the nurses, aides, assistants, social workers, therapists and general staff for such a great experience!

I am extremely disappointed and heartbroken with the care my grandmother received at this facility. When I arrived, she was non-responsive, no nurse was present, and they asked us to leave the room because they were ‘changing her’. They only decided to call 911 after I got there. The EMTs who picked her up said that none of the nurses were in the room, and they didn’t have her medical history or chart available. Horrible staffing and lack of attention contributed to her passing — she passed away just one hour later at the hospital. Families entrust this facility with their loved ones, and this level of care is unacceptable

If I could give 0 stars, I would. My poor grandma has no choice but to be "cared" for here currently. Visiting hours are not posted, though after finally speaking to someone 2 days later they are 8am-8pm- and staff would say that doors are locked after 6pm but someone will buzz you in- after waiting 15 minutes bell after bell? Can't even reach a person by phone! Its disgusting. Her clothes have vanished, which is the primary reason for my visit as I live far from her. Constant disappointment with this establishment and its really disgusting that not only can I not bring my grandma clothes because of my work schedule, but in a week they'd be gone anyway despite her name on tags and seams and everything in between. Truly gross that this is supposed to be a place that cares. I pray she makes it out of here alive. When I finally got ahold of a woman this evening, she hung up in my face after asking me "what do you want me to do about it?" when i tried explaining that I stood there with no help for 15 minutes. I want you people to be mindful of those that are throwing thousands in your face. Its disturbing how obviously little they care. These people do not care about your loved ones nor about you. Absolutely disgusting behavior from adults.

I've been here since about 11/15/24. To date, the most frustrating thing has been getting my meds straightened out and synced up. This seems to be a joint effort between the Hospital and this place. Staff sometimes acts like I'm just out to annoy them when I am struggling with not only suddenly being unable to walk, but with nonstop leg spasms, and side effects from meds. Physical care: I am flat on my back, and need some, er, private care. Some CNAs sail in, bring all the items needed, are pleasant and polite. They open the shades, fluff the pillows, tidy up the obvious clutter, make sure you're comfortable. With these gals, you will likely never get a bed/pressure sore. They won't allow it to happen. I am so grateful for these gals. Others, however, show up an hour after the call button, look disgusted when you make your request, don't bother putting down proper bedding if it's not right at hand, don't use the all-important medicated cream, etc., ARGUE with you when you tell them something about your care When you're flat on your back, this can be very threatening. The caregivers have the power, after all, to make you life miserable. Food: Mostly atrocious. Even for institutional food (I bet YOU'VE never had tasteless bacon before). Medical: There is a robust healthcare tram on board. One of these argued with me over a medication I need to prevent *intolerable* symptoms (not pain) and ordered blood tests which were unnecessary, as i had had multiples the previous week. Watch for updates NOTE: DO NOT PAY ATTENTION TO REVIEWS BY FAMILY MEMBERS. THEY PROBABLY DON'T KNOW THE DAY TO DAY EXPERIENCES OF THEIR LOVED ONES ESPECIALLY IF THEY HAVE DEMENTIA or are unable to communicate.

I had a fantastic experience at Autumn Lake Healthcare at The Willows in Woodbridge, CT. The Aides, Nurses, and many other professionals took care of my every need with kindness, warmth, and constant encouragement. The level of compassion they showed truly made my stay more comfortable. A special thanks to the incredible Physical Therapists, whose expertise and patience guided me from being confined to a wheelchair to walking independently with a walker. Their dedication and skill made all the difference in my recovery. The Willows is fortunate to have such a remarkable team. Thank you again, Linda - 104B

About this data
How to read this page
Star ratings vs. Care Quality Score: The Google star rating reflects visitor and family opinions collected from public reviews. The Care Quality Score is an independent metric derived entirely from federal inspection data — the two measure different things, and comparing them side-by-side gives a fuller picture.
Where this data comes from
All clinical data on this page originates from the Centers for Medicare & Medicaid Services (CMS), the federal agency that certifies and inspects every Medicare- and Medicaid-participating nursing home in the United States. Reviews are imported verbatim from public Google business listings.
