I never write a bad review, but I literally just set up a google account for this purpose. It definitely is not a place to put your loved one in. Our mother was there for only 1 and a 1/2 months and deteriorated drastically before we decided that my sister would take her in. She was very competent, when she went in, but has recently been diagnosed with Alzheimer's and dementia, she forgets day to day, but remembers the past. She's able to dress herself, eat and verbalize. I found her one day because my mom would call me everyday and I haven't heard from her in a couple of days, so I went there. I went in my mom's room with the head nurse Suzanna and we found her completely naked, (with only a pull-up on which she has NEVER wore before) and slurring her words. They even tried to cover it up when I saw it with my own eyes. It was very disturbing. We had to call an ambulance to admit her to the emergency room (not the staff mind you). I called Suzanna and she didn't return my call until 24 hours later. I didn't answer, but wanted to see what she would say when I saw her in person....she ignored me! Imagine the head nurse doing that! It is filthy, smells bad, mold, elevators not working, and human feces on the bathroom floor. I also witnessed human feces in the hallway that the staff had to tell me to walk around it. There was one nurse who was very helpful and knowledgeable about her job, I apologize for not remembering her name. The head administrator never acknowledged or reached out to us about our complaints. The maintenance, kitchen help, administrators, and personal staff have no knowledge of how to take care of our loved ones. They have meetings everyday, but have no clue on patients care, which includes thoughtfulness and compassion for our loved ones. Please stay clear of this facility. I do not understand how this facility is still opened. I feel so bad for the non verbal patients, that cannot advocate for themselves and for all other patients that are not getting the care that they need and deserve.

If I could give this place negative stars, I would. Please, if you care anything about the person being admitted, please, please, please, don't choose this place. It's unsanitary, it's suffocating, mold and mildew is growing around air vents and water sources, there's human waste on the floors and the bathrooms are plastered in it, they leave medication carts and used medical waste out in the open on carts, the phone rings and rings and nobody answers, and if someone does pick up, the staff hangs up on you. We were told patients are supposed to have room phones, but no phone was given so you're forced to call the main number and then you get the run around. It's truly horrifying, and it should be reported and closed down. It's horrific. The smell, the feel, the care, all of it. After only 1 week, my mom went to the ER because they had given her medications without our consent or knowledge (insulin and psychotropics) to the point where she was unable to move or speak. She was found with the door wide open, naked, slumped over her bed, with a fever of 101 and nobody noticed or checked on her until my sister found her in that condition and we called her an ambulance. The lead nurse, Suzanna, tried to cover it up and wasn't even honest about her condition when she was found. Aside from 1 nurse and 1 young girl who answers the phones (who kept apologizing because nobody would pick up the phone on my mom's floor) the staff doesn't care. After an entire month, my mom couldn't even walk because of sores on her feet and overgrown toenails. She started off on the dementia floor with zero services and activities, and went to the basement/mental health floor after she left the ER, but there are no mental health services. It's pure neglect and apathy wrapped up with a decent looking website and promises. I'm sure the place is violating multiple health and ethics codes. I can't believe people are living there.

About this data
How to read this page
Star ratings vs. Care Quality Score: The Google star rating reflects visitor and family opinions collected from public reviews. The Care Quality Score is an independent metric derived entirely from federal inspection data — the two measure different things, and comparing them side-by-side gives a fuller picture.
Where this data comes from
All clinical data on this page originates from the Centers for Medicare & Medicaid Services (CMS), the federal agency that certifies and inspects every Medicare- and Medicaid-participating nursing home in the United States. Reviews are imported verbatim from public Google business listings.
