Several family members have used the rehabilitation program after hospital visits. One family member lives in an apartment with assisted living. The staff is caring, friendly, and easy to work with.

DO NOT GO HERE FOR REHAB OR FOR LONG TERM CARE!! Their physical therapy assistant is harassing patients and pretending she is in charge and emotionally abusing patients as motivation. She threatens that the patient will be sent home, unable to walk, as she claims no progress and then says they can do that and yet when directly asked for metrics so the patient can make progress she refuses to respond or give clear goals. She ignores the patients pain level and expects elderly patients to recover from broken bones and be walking in less than 4 weeks from the point walking was approved. Fails to put braces on right and is giving the patient bruises but refuses to adjust it properly. Does not seem to follow the same school of pt as any actual therapist. Goes a step further and threatens to place patients in long term therapy and stop pt all during what’s supposed to be a single pt session. Her name is Heather. Her license shows she is a pt assistant and patient was told she was a physical therapist. Brings someone named Sara with her who she allowed to initially engage the patient in this threatening manner and then backed Sara up. They’ve been harassing the patient over what insurance she has. It’s a nursing home. It’s on file. The patient even asked if their dropping her in her home or moving her to long term were threats and she was very upset. This is the 3rd time there’s been issues. Someone there came in 2 days in a row saying they were going to take her house and all her assets and all her money. She was very upset and it was a meeting they’d specifically been asked to move to a time I could make. They chose not to. Then they withdrew pt during 7 weeks of non weight bearing even though pt could have been done to strengthen upper body and there were orders for the legs but they refused. It’s still a nightmare. The food looks disgusting. Heather has Sara in doing upper body work on the patient post the 7 weeks but seriously they could have done this while there was nothing else to do but obviously just a money grab. This place is very understaffed and all patients will get covid as it’s been through every month. Only 2 staff per unit of 14 patients per day. And only 1 after 10pm per unit. Very unsafe staffing. Food is served cold. Staff will not warm it up. Patients aren’t given choices on food. It smells and looks horrible. And patients end up with the same left overs for multiple days in a row. Patients are left sitting without call lights in wheel chairs for hours. If they needs their depends changed it can be hours before that happens as staff is horrible at answering call lights. Would not recommend. Worst experience ever. Was already a nightmare with my mom being hurt wanted her in town but really wish she had gone to a swing bed in staples at this point. Didn’t have any of these issues at staples and pt was better. The under staffing is scary and the fact that people fall and are injured and just picked up is scary. Have witnessed many infection control issues at this facility as staff fail to wash their hands going room to room and fail to use hand sanitizer that’s in every room. Not even sure how this is an issue but it is. I watched an rn cough at a patient not cover her mouth then pull a tissue from her pocket that was obviously used blow her nose put it back in her pocket and then go right back to making patient meds. She had covid. So did 4 patients and like 8 other staff within days of me seeing this. I was wearing an n95 and just had a covid shot. I knew they’d just had covid in the unit 2 weeks before. The nurse was taking her mask discomfort out on patients when she had to wear one which unfortunately was not while she had covid. She wasn’t even supposed to be at work being that sick and it being that obvious. But she was very mean to patients when asked why it was because she couldn’t stand her mask and was hot. I have heard multiple other nurses at the facility say they should fall on a wet spot on the floor so they can have 6 months off.
