My dad came in on Wednesday and fell on Thursday, passed away on Friday….. he was there to get stronger. I went to visit him Friday morning and found him unresponsive and I asked how long has he been like this and I was told he’s just sleeping, his vitals are fine so he’s fine

I asked for a copy of the fall report and I was told that there isn’t one…..it wasn’t reported

I had a family member that was here for rehabilitation from a prior surgery. The first time I visited, the front check in tablet didn't even have my family member name in the system and the front desk lady didnt say a word to me, the other three times I was there, no one was even at the front desk and once again, her name was not in the tablet, so I had no way to "check in". Everytime I would visit, I would find the nurses & CNAs on the 2nd floor, either having a side conversation or on their phones in the hallway and several call lights were on. My family member needed help several times (she couldn't put any weight on her foot and she also had a wound vac) and she waited 20+ minutes just to have someone check on her. Which I can understand that there are others at the facility, but when I see them in the hallways having side conversations or on their personal cell phones, it's quite frustrating. The first week of my family member being at this facility - we had a conference to come up with a plan of care. The insurance/office woman was nice, along with the Physical therapist, but my major complaint and the reason why I gave 2 stars is because of the charge nurse during that conference. The charge nurse came totally unprepared and even said she thought it was another resident and even provided that residents name, which is against hippa (I used to be a CNA, so I'm quite familiar with Hippa policy) She knew NOTHING about my family member medical history- just mentioned she thinks she remembered reading that my family member was short of breath over the weekend. Which was correct, but she was supposed to get a chest x-ray the week of arrival due to a deep cough, but it was never done. The nurse said she would get that ordered and mentioned she was going to go review her chart and come right back - she never did. It also took them another 2 days before the chest xray was done and wouldn't even give my family member the results. I mentioned above that my family member had a wound vac, she had to deal with the machine beeping ALL night long because they didnt order the correct container for the wound vac! On top of that my family member went TWO WEEKS without a shower. Everytime she would ask for a shower, thr person would say someone else was going to do it, by the time that person got to her, it would be too late in the day to be done. The facility is clean, but the care needs to improve. I understand that the health care world is extremely short staffed, but when you see the staff fooling around in the hallways and ignoring the residents, combined with the level of care she received - its very sad to see. Along with the clear neglect from the charge nurse. Hopefully my review helps and staff can improve so anyone who comes to this facility can receive better care than my family member did.

This is such a quaint, loving and well-run facility. I was so deeply impressed by the staff and they care they gave to my father while he was rehabbing from Parkinson's complications. PT/OT and ST on staff were also particularly excellent and professional and kind. I felt good about him being there, and this center made a difficult time much easier for all of us.

About this data
How to read this page
Star ratings vs. Care Quality Score: The Google star rating reflects visitor and family opinions collected from public reviews. The Care Quality Score is an independent metric derived entirely from federal inspection data — the two measure different things, and comparing them side-by-side gives a fuller picture.
Where this data comes from
All clinical data on this page originates from the Centers for Medicare & Medicaid Services (CMS), the federal agency that certifies and inspects every Medicare- and Medicaid-participating nursing home in the United States. Reviews are imported verbatim from public Google business listings.
