The manager, Lana, was rude, lazy, and completely unprofessional. She talks a big game about “solving problems” but did absolutely nothing to help. Her attitude was the worst part of my mother-in-law’s stay and reflects terribly on this place. Management like this is unacceptable.

I had a wonderful experience and in particular Jack the nurse and physical therapy staff helped me every step of the way. Only problem as I had Medicare as my insurance and felt I was discharged home too soon as had problems getting off the toilet by myself and mobility without extreme pain.

Had a relative stay here for recovery and the experience was unacceptable. Staff were unresponsive, dismissive, and failed to provide even the most basic level of timely care. Critical needs were delayed, and even essential medications were missed or pushed off with excuses. What they went through bordered on neglect. This facility should not be calling itself “restorative.” I would strongly warn others to look elsewhere.

I would not recommend Allina Restorative Suites in Plymouth. I stayed there for five days post total knee replacement. The P.T. staff were a delight but I cannot say much positive about anything or anyone else. I was advised to arrive prior to 5:00. My daughter and I arrived prior to 4:00 directly from the hospital. We were advised to wait in our car as it would be a liability for them otherwise. After 20 minutes, my daughter and I entered with a walker. The receptionist was very angry we did not wait. Then a nice man arrived with a wheelchair with one footrest. I advised it was okay. I could hold my leg up to my room. He took us to my room, the only private room left. My daughter and I waited and waited. We noticed gum in the floor carpet, lots of hair in the shower drain and dirty window sills. It was 40+ minutes before someone came in and said, "No one has been in yet?" Then she sent in a very elderly man who said he was filling in and admitting three individuals at the same time. He kept repeating himself, was extremely.slow, and said I needed a mantoux test. I agreed. In the five days I stayed there, no one checked the Mantoux test. My daughter had to go get dinner and bring back, as they had closed kitchen by the time they checked me in. The morning after I arrived, their staff brought in a contract for me to sign. I signed it and then noticed the rate that was my responsibility for the private room was different from what was quoted to me. I was quoted a $5.00 a day lower rate than in the contract, as the only room they had for me was a room without a refrigerator. She said, "That's right. I will correct that." During my stay, while guests were visiting me and witnessed me place my dinner order, their kitchen staff took my dinner order but forgot to enter it apparently, as at 5:40 when my daughter went to check on where the dinner was, she was advised all the dinners had been delivered and now the kitchen was closed. Once again she had to go get dinner for me. During my stay, I paged a nurse for my meds, questioning the time had lapsed between my med schedule. The nurse said no, I wasn't due. I questioned. Are you sure? Yes, she said. I am sure. Then one-half hour later she returned with my meds and stated she was incorrect and my meds were overdue. She said she was filling in and this was her first night at this location. I had one regular nurse and when I complained to her, she said the services have really changed since ownership/management changed. When I received my invoice for the five days, it was reflective of the full rate and not a $5.00 a day discount for no fridge in the private room. I called and spoke to someone in the billing and then also the manager of the Plymouth location. Both said the rate was correct. I advised I would be sharing my complaints. The manager said there was nothing he could do about the room rate differrnce of the mere $25.00 difference, but thanked me for sharing my other concerns so they could continue to improve. I find that comical, as the responses here, to the negative reviews, ask individuals posting negative reviews to call the manager. I did that first, hoping to not have to post a negative review. Please carefully consider any rehabilitation stay at this Plymouth location carfully. I wish ai would have. The stress caused by staying there certainly did not improve my recovery. The only positive was the P.T. staff.

This rehab place used to be terrific before Allina took over. Clearly very short staffed.

How to read this page
Star ratings vs. Care Quality Score: The Google star rating reflects visitor and family opinions collected from public reviews. The Care Quality Score is an independent metric derived entirely from federal inspection data — the two measure different things, and comparing them side-by-side gives a fuller picture.
Where this data comes from
All clinical data on this page originates from the Centers for Medicare & Medicaid Services (CMS), the federal agency that certifies and inspects every Medicare- and Medicaid-participating nursing home in the United States. Reviews are imported verbatim from public Google business listings.
