The care team here is impeccable! My brother has been so happy and taken care very well here both in the rehab section and most recently in the LTC floor.

My mom stayed at St. Anthony Rehab Center for just over a week after being released from the hospital. She has late-stage dementia, and during the Christmas holiday this was the only facility able to take her on short notice. What felt like a desperate and overwhelming situation turned into something far more meaningful because of the people who work here. The staff at St. Anthony are pure gold. They are not just caregivers — they are compassionate, patient, and deeply human. They treated my mom with dignity, tenderness, and respect at a time when she was completely vulnerable. As a daughter, trusting others with your parent’s care is heartbreaking and terrifying. This team gave me peace. I will always be grateful for the love and kindness they showed my mom and our family during one of the hardest times of our lives.

Food is unappetizing and cold. Care is substandard. Lots of texting for hours not minutes at the nurses station. Three nurses watched me move my mother to another room without assisting. Charting system and TRA staff communication are substandard. One of us is there every day while our mom is there to ensure her safety. We will be getting her out as soon as we can get home nursing and therapies arranged in her home.,

About this data
How to read this page
Star ratings vs. Care Quality Score: The Google star rating reflects visitor and family opinions collected from public reviews. The Care Quality Score is an independent metric derived entirely from federal inspection data — the two measure different things, and comparing them side-by-side gives a fuller picture.
Where this data comes from
All clinical data on this page originates from the Centers for Medicare & Medicaid Services (CMS), the federal agency that certifies and inspects every Medicare- and Medicaid-participating nursing home in the United States. Reviews are imported verbatim from public Google business listings.
What to do next
Data is a starting point, not a final answer. We encourage families to visit facilities in person, observe daily routines, and speak with staff and residents. See our guide: Questions to Ask When Visiting →
