⭐ 1 star — Avoid at all costs

If I could give minus 5 stars, I would.
My mother was a resident at Sunset, and the care she received there was shockingly negligent. What ultimately happened to her was not inevitable...it was the result of systemic failure, lack of basic care, and a complete absence of accountability.

From the beginning, it was clear that residents are treated like tasks, not people. My mother received little to no meaningful physical therapy for long periods of time, despite clear signs that movement and repositioning reduced her pain. Instead, she was left immobile for long periods, which led to severe decline, worsening pain, and ultimately serious pressure wounds that should never have progressed the way they did.

Nutrition was another constant battle. Unless family was physically present to advocate, encourage eating, and monitor what was actually consumed, she simply did not get adequate nutrition. This contributed to malnourishment, weakness, confusion, and repeated infections due to inadequate pericare.

Communication was abysmal. We were repeatedly told things were being “monitored,” yet no follow-ups ever happened. Cultures were not repeated. Symptoms were dismissed. Concerns raised by family were ignored until they became emergencies...at which point she was sent back to the hospital time and time again. This became a revolving door: hospital stabilizes her → Sunset neglects her → she deteriorates → back to the hospital.

Staff turnover was constant. Many caregivers were traveling nurses unfamiliar with residents. There was no continuity, no ownership, and no sense that anyone truly knew or cared about her baseline condition.

The most devastating part: the lack of basic care at Sunset directly contributed to my mother’s rapid decline and eventual end-of-life situation. She deserved dignity, attentiveness, and competent care. Instead, she was treated like a liability to be managed rather than a human being to be helped. The only thing they managed to do was make her worse in every capacity.

This facility is expensive, yet delivers pathetically substandard care. If you love your family member and expect even baseline attention, advocacy, and follow-through, do not trust Sunset.

We trusted them. And we will live with the consequences of that decision forever. Avoid this facility AT ALL COSTS.

My brother was placed there for end of life care. I assumed it included comfort care. The place is seriously understaffed. Why they keep excepting new patients when they do not have the staff to care for them, is beyond me. Simple requests to the staff about his care by family, was positivity responded to but not carried through. On his last day I went up to see him. His lunch tray was by him. The enchilada was cut in half. Containers with sealed lids were not opened. This is after they had agreed to that food would be cut up into small pieces and à staff member would be with him to assist him. He was laying on his bed with just an adult diaper on and no covering on him. They say he went to the dining area for supper and when they took him back to his room, he requested to stay in his wheelchair and when they came back he was gone. This is total bullshit. He was too weak and barely verbal when I saw him. He would not have requested either of those things. This was just his last day. The doctor who saw him that morning wrote that he was at end of life and family should be notified. We weren't and he died alone. Shame on you!!!

My father was at Sunset Drive GOOD Samaritan for only a few days. He fell out of bed. No precautions were put in place such as a fall mat beside his bed after his fall. He fell out of bed again today and hit his head. No fall mat was put in place. He is on blood thinners and is only at this "care center" because of a stroke he suffered from just 16 days ago.
I am VERY UNIMPRESSED by this facility at this point. As a former ND SATE HEALTH FACILITIES SURVEYOR, I know exactly how nursing homes should be run.
THIS facility is already failing my father.

My Grandma was there at the end of her life. This facility and staff were amazing. Not only did they care for my Grandma like their own, they also cared for us, that were with her pretty much 24/7. I can not say enough good things about this facility. would highly recommend!

About this data
How to read this page
Star ratings vs. Care Quality Score: The Google star rating reflects visitor and family opinions collected from public reviews. The Care Quality Score is an independent metric derived entirely from federal inspection data — the two measure different things, and comparing them side-by-side gives a fuller picture.
Where this data comes from
All clinical data on this page originates from the Centers for Medicare & Medicaid Services (CMS), the federal agency that certifies and inspects every Medicare- and Medicaid-participating nursing home in the United States. Reviews are imported verbatim from public Google business listings.
