Let me first say, that most nurses were ok. Rooms beautiful. Sheets were stained and looked dirty. Hadn’t been changed. Found a bed bug crawling up the curtain, a few more in the corner of room on the floor. Food crumbs all along the floors and under the bedside tables. That being said. With of the things we saw? We had to move our grandmother to a different facility across town. All the reviews on here? No WAY can those be accurate. We recognized 3/4 of the pictures of people working here. A team is only as good as the weakest link. The administrator, is their weakest link. Because if she was taking care of all these issues? We wouldn’t be seeing all of the dirty conditions. The administrator kept avoiding us and our all our concerns. Door was always shut. We never saw the doctor except through a phone. Diversify your medical knowledge but don’t go here! Thank goodness we moved her when we did, or we might’ve lost her.

Worse I could give them! The administration is hands down the worst I have ever seen! I have seen drug and alcohol treatment centers that have way better administration…. Biden has. Better administration 🙄 🤦♀️

The reason I only gave three stars is their administration is ridiculous! When my mother was admitted for rehab she was approved in the door for 100 days no questions asked! My mother has dementia, she had a very bad wound where they took her tissue down to the muscle. That happened the third week of July! My mother smokes a lot. She wanted to smoke (keep in mind she doesn’t think clearly wen reasonably) the staff AND case manager and administrators knew this. Supposedly she had to walk 150 feet unassisted and be able to shower plus a few extra little goals. She threw a fit after I wouldn’t take her home one day and showed her butt to them saying I want to go home blah blah. She wasn’t even off the wound vac a week at that point! The wound IS still open: she was there 21 days maybe 25 (of the 100) full paid!!!! She came home with home healthcare! Here’s the icing on the cake my mom had a stroke last night! So thank you Piqua manor for sending her home when we kept telling you it was too soon! She had plenty of days left: y’all have never dealt with a scared dementia patient?! You shipped her out the door so she was outta your hair. I hope this doesn’t come back to you guys in some way 🤷🏻♀️ also all the aides and staff that worked with her hands in were great! If you’re gonna send your family there stay on top of their clothes they lose people’s clothes even though I was washing them myself. Make THAT make sense! One last thing mom said their food was good and I did see a lot of activities for those who could partake in them. I feel they should do activities with their more bed ridden people too! Just a thought 💅 🤔with in ten days of release my mom had a stroke. She came home last Thursday and had it somewhere between last night and this morning! 🗣️🗣️🗣️🗣️🗣️🗣️

About this data
How to read this page
Star ratings vs. Care Quality Score: The Google star rating reflects visitor and family opinions collected from public reviews. The Care Quality Score is an independent metric derived entirely from federal inspection data — the two measure different things, and comparing them side-by-side gives a fuller picture.
Where this data comes from
All clinical data on this page originates from the Centers for Medicare & Medicaid Services (CMS), the federal agency that certifies and inspects every Medicare- and Medicaid-participating nursing home in the United States. Reviews are imported verbatim from public Google business listings.
What to do next
Data is a starting point, not a final answer. We encourage families to visit facilities in person, observe daily routines, and speak with staff and residents. See our guide: Questions to Ask When Visiting →
