My grandmother was a resident for four months while recovering from a broken leg. Both she and I have extensive healthcare experience across different healthcare settings, so we have a clear understanding of standards of care as well as the challenges frontline staff face. The nurses, aides, and rehabilitation staff worked hard under fast-paced, understaffed, and demanding conditions, and their dedication was evident. Unfortunately, managementβs reliance on agency nurses raised concerns, including one instance where my grandmother was delivered a meal tray and not checked on again for the remainder of the shift. Discharge planning support was also limited. As a caregiver, I received minimal guidance from the social worker and was frequently directed to seek information on my own. While I was able to navigate this due to my background, this lack of support would be difficult for families without healthcare experience. Additionally, there was no follow-up after discharge to ensure a safe transition. Overall, the experience felt focused on turnover rather than continuity of care. Pros: β’ Dedicated and compassionate nurses, aides, and rehabilitation staff who worked hard despite being overextended and understaffed β’ Strong rehabilitation support that contributed positively to recovery β’ Frontline staff showed genuine care and effort under challenging conditions Cons: β’ Heavy reliance on agency nurses led to inconsistent care and lapses in monitoring β’ Limited support and guidance from the social worker during discharge planning β’ Caregiver was often expected to independently coordinate next steps β’ No follow-up after discharge to ensure a safe and smooth transition

About this data
How to read this page
Star ratings vs. Care Quality Score: The Google star rating reflects visitor and family opinions collected from public reviews. The Care Quality Score is an independent metric derived entirely from federal inspection data β the two measure different things, and comparing them side-by-side gives a fuller picture.
Where this data comes from
All clinical data on this page originates from the Centers for Medicare & Medicaid Services (CMS), the federal agency that certifies and inspects every Medicare- and Medicaid-participating nursing home in the United States. Reviews are imported verbatim from public Google business listings.
What to do next
Data is a starting point, not a final answer. We encourage families to visit facilities in person, observe daily routines, and speak with staff and residents. See our guide: Questions to Ask When Visiting β
