Ms. Baker, Social Services caseworker; did an outstanding job working with my brother's discharge plans. She was prompt and thorough. Ms. Baker listened well and showed genuine concern for his specific needs. Am very grateful to her!!

Terrible place!!! So sad that this was the only place that accepted my dad’s insurance. The place smells awful and the employees are rude as anything! They don’t really care for you or your family. They hate their job. Granted it’s a depressing place, but please treat the patients with respect and kindness. My dad got transferred to here and we had to get him out of this horrible place immediately. Even my mom asking for a box of tissues was looked down upon from the worker there! If you want your loved ones to die in a horrendous place and miserable then this is the right place for you. It’s like a ghost town. They need to be inspected. Everything is run down like an abandoned asylum. Don't trust the pictures!!! It’s worse than that. They need to be shut down or totally replace staff. Please treat patients and family with the respect they deserve or don’t be in this field. Save your loved ones with some sanity left. My dad wanted to leave immediately. Got him out the same night he was brought in. Just to give you a glimpse of how horrible it was for him. They didn’t even care to give him medicine or anything for hours! The nurses/staff they are loud, rude, careless and just enjoying getting paid by doing nothing for their patients. Listening to their tablets and eat and drink. Again if this sounds like your cup of tea, then by all means. Never again!

Five star review for Ms. Emirror Baker, who assisted me with my father's discharge and filing an appeal with the insurance company. Ms. Baker knows the processes like the back of her hand, and is incredibly effective in coordinating the logistics across multiple providers within Annandale Healthcare Center and the external providers like the insurance company, the transportation company, and post-discharge staffing service company. She is responsive to texts and does not hesitate to call whomever needs to be contacted to ensure plans are made and everyone is informed. My parents have been in and out of hospitals, rehabilitation centers, and assisted living for the past five years, and Ms. Baker is the best person with whom I have worked. I cannot thank her enough for her assistance. My father was a patient on Emerald Court and he was well cared for by the nurses, physical therapist, and speech therapist. I wish there was an easier way to get updates either by direct call or online portal, as it was sometimes frustrating to not be able to reach nurses directly - this is an area for improvement to get to five stars. All staff with whom I interacted while visiting with my father were professional, helpful and kind.

I would not recommend sending your loved ones here. The staff does not seem capable of providing proper care. The patient was not monitored adequately while under doctor’s orders, and it took over a week for anyone to notice that the patient had developed an infection.

Throughout the process, Ms. Baker demonstrated a high level of professionalism, compassion, and attention to detail. She consistently ensured that the patient’s needs were met promptly and with genuine empathy. Her communication skills were exemplary—she maintained clear and timely updates with both the patient and the wider care team, fostering trust and collaboration. Ms. Baker also showed strong clinical judgment and initiative, anticipating potential concerns and addressing them proactively to support the patient’s well-being. Her commitment to delivering high-quality care reflects her deep sense of responsibility and integrity. Ms. Baker’s contribution made a significant positive impact on the patient’s experience and on the overall effectiveness of the care team.

How to read this page
Star ratings vs. Care Quality Score: The Google star rating reflects visitor and family opinions collected from public reviews. The Care Quality Score is an independent metric derived entirely from federal inspection data — the two measure different things, and comparing them side-by-side gives a fuller picture.
Where this data comes from
All clinical data on this page originates from the Centers for Medicare & Medicaid Services (CMS), the federal agency that certifies and inspects every Medicare- and Medicaid-participating nursing home in the United States. Reviews are imported verbatim from public Google business listings.
