My mother was transferred to Skyline for rehabilitation from Forsythe Medical Center in Winston Salem, NC. I had spoken with the person over admissions prior my mother being placed at Skyline.

I was assured the physical and occupational therapy was excellent and they would work with my mother daily to help build her strength and get her to being able to stand and move using a walker. I could not be there when my mother arrived, but I saw the condition she was in when she left NC. Each daily I spoke to the unit manager who assured me that she was doing great. I also spoke with my mom every day. At the beginning, she told me she was doing the therapy and was feeling better.

Nine days in, my brother & niece found her laying on a steel bed frame because the mattress had a defect and wasn’t inflated. She was laying in soiled bed clothes and had soiled herself. She had skin tears on her backside from laying on the bedframe for who knows how long. Her physical appearance was appalling. Even though I was told the on call Dr had seen her, she was having a serious reaction to a medication she was given. She was so weak she couldn’t even stand. The top layer of skin on her entire body was peeling off, and her arms and legs looked inflamed, miscolored,. It appeared she suffered from 2nd degree burns. She was in a great deal of pain. I demanded she be transported to a hospital. She had spent 6 days fighting for her life. It will be months before Mom’s skin will heal it self.

I’ve never encountered such a poor excuse for a facility that is responsible for the care of human beings. My mom has suffered long term effects due to the neglect she experienced at Skyline. I would not recommend this facility to anyone after seeing how my mother was treated, appearances are definitely deceiving.

The staff is fantastic! They are compassionate and patient. My parent is not an easy person to care for, but she truly enjoys the staff here. My 4 stars is because the facility is dated and I wish they would/could update it.

About this data
How to read this page
Star ratings vs. Care Quality Score: The Google star rating reflects visitor and family opinions collected from public reviews. The Care Quality Score is an independent metric derived entirely from federal inspection data — the two measure different things, and comparing them side-by-side gives a fuller picture.
Where this data comes from
All clinical data on this page originates from the Centers for Medicare & Medicaid Services (CMS), the federal agency that certifies and inspects every Medicare- and Medicaid-participating nursing home in the United States. Reviews are imported verbatim from public Google business listings.
