Hello world! I am a resident at Crossroads Care Center in Fond du Lac, Wisconsin. After three nights and on the 4th morning I decided I liked this place enough to cancel my request for a State Veteran’s Nursing Home. I am very comfortable here. The staff is most qualified, the building and grounds are acceptable, the food is palatable, the residents are each pleasant . Housekeeping cleans and mops my room daily. Smiles abound here. The personnel are friendly, timely and experienced. I’m here for the long haul and from my perspective I can honestly recommend to everyone Crossroads Care Center.

Unsolicited and honest,
Walter O.

My grandmother is there. Nurse was fast. Not the friendliness of place but it was the fastest call light answered that I have ever seen and I worked in nursing homes. It is very dirty though. The bathroom in the 2nd floor bathroom was very gross, it had poop on the toilet seat.

Our family received excellent care here. Crossroads had the best care of the three facilities he was at. Thank you for your care, concern and effort. We will always be greatful.

I do NOT recommend this place whatsoever!!..The reviews are very deceiving! The place is very dirty and very smelly!.The care my loved one received was terrible! Call lights took over an hour to answer, where I would get calls at home stating he was in tremendous pain and Noone would answer his light, where I had to actually go in to answer his light and take care of his needs.I was EXTREEMLY UPSET!...I did have meetings with the DON,ADON,SOCUAL WORKER.Even had to let State know what was going on...I could literally write a book of all my concerns. He was told several times when needed the urinal to just wet his diaper and they will just change it,which was a huge dignity issue for him.I got many phone calls daily with him complaining to me of horrible care.I would have taken care of him at home but he was more care then I could do, seeing he required 24hr care. He was on hospice.I spend sometimes most of my day by him there just so I could take care of him.Like I said I could say so much more!!..PLEASE BE AWARE OF THIS PLACE!!..tour it before you decide and make for certain you dont just check out the first floor, check out the 2nd floor too. I am a CNA at a different facility where I live but there was no room for him ,I do know the Nursing field well,Ive been a CNA over 32 years which was a Great advantage in the case of my Loved one.I do know what is expected in these Nursing homes and in my own opinion I can not see how this place can even stay open!! PLEASE USE CAUTION AND DO YOUR RESEARCH!!

About this data
How to read this page
Star ratings vs. Care Quality Score: The Google star rating reflects visitor and family opinions collected from public reviews. The Care Quality Score is an independent metric derived entirely from federal inspection data — the two measure different things, and comparing them side-by-side gives a fuller picture.
Where this data comes from
All clinical data on this page originates from the Centers for Medicare & Medicaid Services (CMS), the federal agency that certifies and inspects every Medicare- and Medicaid-participating nursing home in the United States. Reviews are imported verbatim from public Google business listings.
