My father has been here for 2 years, care is mediocre as they are understaffed. Many of the staff do try to provide good care, but as with any facility there is quality staff and people who shouldn’t work in this field. Just a few recommendations to those who have family in a Nursing home…..including this one.
* You are their advocate! Do not let them talk you into making your family member a DNR regardless of what they tell you or promise you that their care will not change…. It’s a lie. This takes the burden off the facility if something should happen to your loved one.
* Care conferences - take your own notes (as they do not), no one will get back to you on questions or concerns (as they assure you they will). Blame is always placed on someone that is not in the room. Again…. You are their advocate…..be persistent with their care.
* Recently they have had their phone service cancelled, you cannot reach your loved one without having staff go get them or letting them use staff’s personal cell phones and your loved ones cannot reach you without going through the front desk phone or utilizing staffs cell phones which takes the staff away from their other duties in a facility that is already under staffed.

My Sisterin law lived there for 6 years and enjoyed it very much. She was well taken care of in her last days and treated with dignity. Thank You very much!

This place is extremely unprofessional and disgraceful. None of the staff is on the same page with the progress or status of a patient. They use the excuse that they're short staffed for not having "time" to bathe the patients on a regular basis but they have at least 7 "nurses" having a conversation at the front desk while residents alarms are going off- and no it was NOT shift change. The employees act lazy, inexperienced, and are extremely rude. It's almost like they have a title and name tag on their work shirt for someone who is unqualified for the position and will hire just about anyone. My recommendation is to NOT put your loved one in this specific place.

Do not let any of your loved ones stay here! UNLESS you plan on being there in the morning and at night to take care of their daily routines because they will not. They will let your person stay there in their room, alone, doing nothing all day except for the "peek in" visits every few hours for maybe a minute. They also left medications unattended in the client's room for any person to come in and take and then the nurse argued with me that I was wrong. I am trained medical professional so I know. 3 Oaks is just as bad if not worse! I used to get clients from them with bed sores so bad it would take a year to recover from them! They definitely do not deserve a National Quality award!

About this data
How to read this page
Star ratings vs. Care Quality Score: The Google star rating reflects visitor and family opinions collected from public reviews. The Care Quality Score is an independent metric derived entirely from federal inspection data — the two measure different things, and comparing them side-by-side gives a fuller picture.
Where this data comes from
All clinical data on this page originates from the Centers for Medicare & Medicaid Services (CMS), the federal agency that certifies and inspects every Medicare- and Medicaid-participating nursing home in the United States. Reviews are imported verbatim from public Google business listings.
