Place is dirty and bathroom had poop stains and crusted urine. Staff was unaware of why my girlfriend’s stepfather was in there for. The nurse was recorded specifically saying he didn’t know why her stepfather was in there for which was for rehabilitation of his walking capabilities (which wasn’t provided he was kept in a wheel chair instead) they supposedly called police for recording and talking to another resident patient for asking permission to record them also they had no signs stating recording was not allowed in the building but cops never showed anyways. I was tone checked by a lady (staffer) there when I was simply trying to make the statement that we had the same complaints as the one star reviews on google here. What she was trying to do was make sure other visiting families didn’t hear what I had to say or tell them to check the reviews on google. The one star reviews are the same complaints we had and I’m letting people know that staff got rude with me for that fact. Don’t send your loved ones here they don’t keep track of their patients’ health situation which is what I realized when the nurse didn’t know why my girlfriend’s step father was in there for. Also as we were leaving we talked to another family who had the same complaints stating the facility also didn’t no why there sick family member was In for which was for MS very severe condition is what we were told and they were not happy at all and waiting on family member to come help handle the situation.

We were very disappointed in the care my husband received at Crossroads. Very unprofessional staff and poor care provided.

I would actually give it a zero. The bathroom was filthy. The bed was metal very uncomfortable and I actually waited in my wheelchair for 4 hours to get help. I was not supposed to be in the chair for longer than an hour. It was very hot in the room they did not have fans. The window had a huge hole in the screen so we did not open it. My daughter helped me to the bathroom with my chair, she had to clean the bathroom first. I was supposed to have a walker I never got one. I asked about the menu or food because I have allergies. I was told I could eat a hot dog. We waited about 4 hours and we left. There was no way my daughter was going to leave me there. Of course there's more negatives but what I have said is bad enough. Very dirty. I have two friends there in the past few months and I won't even go visit them. I felt traumatized

My mom came here for rehab after suffering a fall and stayed for about 2 months. She loved the meals and the staff. As the activated POA, I was very happy with the communication of the staff and the excellent care she received.

About this data
How to read this page
Star ratings vs. Care Quality Score: The Google star rating reflects visitor and family opinions collected from public reviews. The Care Quality Score is an independent metric derived entirely from federal inspection data — the two measure different things, and comparing them side-by-side gives a fuller picture.
Where this data comes from
All clinical data on this page originates from the Centers for Medicare & Medicaid Services (CMS), the federal agency that certifies and inspects every Medicare- and Medicaid-participating nursing home in the United States. Reviews are imported verbatim from public Google business listings.
What to do next
Data is a starting point, not a final answer. We encourage families to visit facilities in person, observe daily routines, and speak with staff and residents. See our guide: Questions to Ask When Visiting →
