Although I donβt have a family member within this facility I do visit with a friend occasionally when she sees her great aunt. This place seems so caring and compassionate. I heard it has gotten a bad rep in the past but they appeared to have done a complete 180 (told was due to newer director & mgt personnel). It is warm & inviting and majority of the staff is always smiling and friendly. Should I ever have to place a loved one in a nursing home/senior care facility I would definitely start here. Thank you to all the CNAβs working hard there as I was also one in the past. ππ«ΆπΌβΊοΈ

We always speak the truth when we say we are so fortunate that Mom is in your care. β€οΈ Your entire staff work in such unity together to bring such excellent care to your residents...from kitchen staff to nursing staff to support staff, your sweet Activities Director - truly a family atmosphere. Thanks for all you do each & every day to make us feel that Mom is exactly where she is meant to be. We feel at peace knowing our Dear Mother is in such good hands. In the beginning of this journey, it took such a long time to find placement for Mom. You had no openings for a long period of time until just one bed became available - I believe Mom was meant to fill that space and to be there with you.<br>Love your fall decor out in the lobby & on your bulletin board - it's all of your acts of kindness that add up to keep our hearts π full of gratitude.

We are so grateful for this place and the WONDERFUL caring loving staff who take care of my Grandma. We are always so impressed with how well they stay on top of her care, how they offer extra loving ways that make all the difference, and how excellent the staff is at communicating with all of us, as well as implementing any requests we have. We waited a long time for an opening here to become available and are so glad we waited it out so she could have this exceptional care that she so deserves. Thank you Tomahawk Health Services for everything you do to make my Grandmaβs life as full and comfortable as possible. We Love and appreciate you so much. You are Angels on Earth & we are so grateful to be blessed by your hard work and dedication.

My step dad is a patient here. He is in rehab so he can go home. He has some dementia, but not enough that he can not function at home with my mom. The nurse practitioner prescribed him lorazepam 2 times a day due to one issue where he was agitated. They had already lost his hearing aids after 1 week of being there, so he gets confused because he can't hear anyone. He was SNOWED on this med. PT and OT could not work with him because he couldn't even name an animal. They brought their concerns to my mom, and could not understand what happened. He was fine prior to this med. My mom and I went to her requesting this be discontinued. She REFUSED and threatened to make my mom take him home that day if it was stopped. Her words were "I won't do that on a 3 day weekend". This was on Friday before Memorial Day weekend. I informed her that it is illegal for her to refuse stopping the black boxed med at my moms request, and that this med is not meant for dementia behavior. She has intimidated my mom into agreeing w/ inappropriate treatment, and when I wouldn't put up with it she threatened to call the police. I told her go ahead - because she is illegally drugging my step dad. She then called a doctor that she works under (Dr Hagenstein) I believe was his name. He was wonderful to my mom over the phone. He AGREED with my mom and I, and took him off the lorazepam. He adjusted a different medication, and the NP was told the lorazepam will be weaned off by the end of this week. As of yesterday he was almost back to his self prior to going into the rehab. This NP should NOT be treating patients. She is illegally medicating patients so they are not alert. I will be calling the state for them to investigate.

About this data
How to read this page
Star ratings vs. Care Quality Score: The Google star rating reflects visitor and family opinions collected from public reviews. The Care Quality Score is an independent metric derived entirely from federal inspection data β the two measure different things, and comparing them side-by-side gives a fuller picture.
Where this data comes from
All clinical data on this page originates from the Centers for Medicare & Medicaid Services (CMS), the federal agency that certifies and inspects every Medicare- and Medicaid-participating nursing home in the United States. Reviews are imported verbatim from public Google business listings.
What to do next
