My grandfather was here for rehab after a fall; at his stay he was able to heal as quickly as possible. The staff there were so friendly and my grandpa loved them. I would 100% recommend Four Winds!

Do not put someone you love in this facility.

At a price tag of almost $15,000 a month, basic needs (like proper hygiene) were consistently neglected. My grandmother developed multiple UTIs due to poor incontinence care, and the response (simply administering antibiotics) was consistently so absurdly delayed that the most recent infection progressed to sepsis. She was hospitalized and did not recover.

This level of negligence is unacceptable. Families entrust their loved ones to these facilities expecting compassionate, competent care. What we received was neither.

Walk-through yesterday to check out as a Short-term rehab facility for my friend postop total knee. Very dingy/depressing no receptionist on the weekend no answering machine no information available somewhat skeptical to go there.
Heard, the food is not great. That’s an issue… Going to check out Ingleside in Mount Horeb.

About this data
How to read this page
Star ratings vs. Care Quality Score: The Google star rating reflects visitor and family opinions collected from public reviews. The Care Quality Score is an independent metric derived entirely from federal inspection data — the two measure different things, and comparing them side-by-side gives a fuller picture.
Where this data comes from
All clinical data on this page originates from the Centers for Medicare & Medicaid Services (CMS), the federal agency that certifies and inspects every Medicare- and Medicaid-participating nursing home in the United States. Reviews are imported verbatim from public Google business listings.
What to do next
Data is a starting point, not a final answer. We encourage families to visit facilities in person, observe daily routines, and speak with staff and residents. See our guide: Questions to Ask When Visiting →
