Although I'm a Devout Lutheran; I am shocked & deeply distressed to share that my personal observation & detailed experience regarding the in-house 'lack of appropriate care' for basic referral Rehab Services of my "Beloved Mother" was well-below par!<br>My claim of total & complete inadequate care can be substantiated by an array of photos, video tracking and subsequent demand calls for an Administrative Consultation Meeting of Staff, after numerous voices concerns regarding her stay went unaddressed in a timely fashion. Ultimately; the sub-standard (POC) "Plan Of Care" eventually attributed to her ambulatory return & re-entry back into a hospital facility where she progressed to ICU and her unexpected Death!<br>Unfortunately; I am saddened to say......<br>based on my 1st hand, direct knowledge I encountered as my Mother's Caregiver, overseeing her status, I would NEVER again suggest, recommend, advise nor promote this facility!!!

My husband fell & broke his left arm in September. He was hospitalized for 11 days for surgery & recovery. The hospital's social worker suggested two rehab facilities and I chose The Lutheran Home as my mother had stayed there briefly after a surgery.<br>He stayed at The Lutheran Home for almost 18 days. During that time he worked with a PT therapist, an OT therapist, a Swallow therapist and was provided a special meal accommodation as of course he fell while in the midst of dental procedures for new dentures.He was toothless.<br>After experiencing a lack of care at the hospital he was in poor physical shape. I was very impressed that he was seen by a series of therapists within an hour of his arrival. A plan for his therapy was in place the next day and was revised based on his physical ability throughout his stay.<br>What impressed me is every decision was shared with me .In addition I felt for the first time in weeks the staff was listening to my input. After all I knew<br>what he had experienced at the hospital , or what medications he was on or what additional health issues he has.<br>I felt he received the best care and attention from every staff member who entered his room. I was greeted by staff daily when I visited. It was such a warm ,welcoming experience after his hospital stay.<br>If you are faced with the difficult question as to where will my loved one receive the best care he'she needs in recovery, I highly recommend you consider The Lutheran Home.

My mama came here for her rehab 10 plus years ago I do not know what has happened but this place is HORRIBLE HORRIBLE HORRIBLE. I came 2 hours ahead and the room looked like someone just left which we found out they did. The lady name was Barbra no one inspected the room . The bed did not work the chair did not work the floors were dirty and there was toothpaste left in the bathroom.The social worker does not communicate with residents it’s all about money and percents which working in health care 23 years rehabbing and doing what’s best for the person is first ! I have worked in admin for a long time and I have never seen this type of dysfunctional environment. The CNA ‘s some were really awesome like Mary off of 1 st who has been there a long time and others are allowed to talk to the residents like crap and like they are not adults. The residents rights are out the window at this place the resident has the right to know about their plan of care and what is on their care cards THEY DONT EVEN HAVE THESE! The nurses do not give report on the resident. The first 2 days NOONE knew my mom was in the room I had to come and stay. They sending her home knowing they have not even went over stairs and she has 14 at home and also getting in and out of cars she has not even did. They document false things saying she was eating 50 percent smh that is all a lie on top of documents saying they did stairs and they absolutely did not the did this literally 2 days before throwing her out! She could only do 1 stair. They are only about money no matter what these people say they have lost they way in how they care for a person
