WARNING: Neglectful Care – Residents at Risk
I am writing to warn families about the care at Diversicare of Pell City. My father was repeatedly left in the same position for long periods, and staff failed to turn him or provide basic care. This neglect caused a deep wound on his buttock, and we ultimately had to move him into hospice care to get proper attention.
Residents’ basic needs, comfort, and health were clearly not being prioritized. Families need to know the serious risks of leaving a loved one here.

If there was an option for negative rating, that's what I would have chosen. Family member has been in this facility several months for what was supposed to be a short rehab stay, the temporary stay turned into long term after rapidly declining from the neglect he has received at this facility. Medications are not given as directed or ordered due to the nurses failing to get orders turned in for days at a time. A wound that requires wound care at least 3 times a week due to the nurses not following orders to redress and clean. Nurses pass blame from one to another. Patients rooms are filthy and not cleaned. Rooms are not cleaned between moving one patient in and one patient out. Patients are scolded for crying out for help when their roommate falls from the bed, while the nurses congregate and chat in the lobby area at night. The administration doesn't take accountability or try and resolve issues. I do not recommend this facility to anyone! Do not allow your loved ones to be neglected by this staff. The state of Alabama needs to investigate and, I will be taking the proper procedure to ensure that happens!

Terrible facility. My mom is 90% blind and deaf. She was here for a 21 day rehab. I had heard the horror stories about here, but it was close by. We showed up unannounced on a Saturday evening about 7:30pm for a check on mom. When we got into her room, it smelled of Urine and feces. Mom was soaked and her bed was dripping wet with urine. You have obviously been there all day and has not been changed or checked on . all the workers were at the desk, laughing and on their cell phones. Me and my wife changed her diaper and the bedding ourselves. On another occasion, I dropped in for lunch, and mother was sitting there with a full plate of food that had not been touched. She said she didn’t even know she had a plate of food because she has blind and cannot see and they knew this, but yet they would not feed her or help her with her food and then they would come back and just get the tray without checking to see if she’s eaten. The horror stories are true about this place. run away and don’t put your loved one here.

My mother had Parkinson’s disease as well as dementia when we took her here in 2020. The hospital before this had her eating again and getting back to semi-normal. After a while of being in this facility she started losing weight dramatically keep in mind she was already around 100 pounds if not less. The doctor gave her two high doses of Ritalin which is a medication for ADHD which she did not have. It made her condition even worse and she lost more weight and mentally declined not to mention physically harming herself by accident. When she died she looked like a skeleton. Her death certificate literally says she died from malnutrition. They neglected to keep her fed and seems like they were just giving her medication for the hell of it. It was haunting to see her like that especially since I was 16 at the time. I highly recommend choosing somewhere else to entrust the wellbeing of your loved ones.

I wish I could give zero stars. I have a family member in this place and she has had medical emergencies where she has been transported to the hospital from this place and not once have they called the people they were supposed to notify. They are awful

About this data
How to read this page
Star ratings vs. Care Quality Score: The Google star rating reflects visitor and family opinions collected from public reviews. The Care Quality Score is an independent metric derived entirely from federal inspection data — the two measure different things, and comparing them side-by-side gives a fuller picture.
Where this data comes from
All clinical data on this page originates from the Centers for Medicare & Medicaid Services (CMS), the federal agency that certifies and inspects every Medicare- and Medicaid-participating nursing home in the United States. Reviews are imported verbatim from public Google business listings.
