IF I COULD GIVE 1000 NEGATIVE STARS I WOULD!!! This is the ABSOLUTELY WORSE facility for patients who need rehabilitation I have ever seen. My dad left the hospital on a Thursday. We went 3 times (2hr drive 1 way) ourselves (Mom and I ) and had other family members there on the days we couldn't be. He was NEVER showered, never was able or helped to brush his teeth. When they brought his meals, they didnt even push the tray up so he could get to it. Him and the patient in the same room would push the help button and it would take at a minimum of 30 to an hour for them to respond when he needed to use the bathroom. He had fallen before hospital stay (for both lung pulmonary embolisms) and had a big 6x4" raw skin peeled back. And they had not checked it, or changed the bandage. Their reasoning, IT WASNT ON THE MEDICAL REPORT FROM HOSPITAL) SERIOUSLY. Like they couldn't see a huge bandage and ask him. He was dirty and smelled awful. The rehab was 30min on a hand/foot bike. That's it. If you have a family member who needs rehab. DO NOT USE THIS FACILITY!!! He has parkinson, so needed help with every day things. They didnt help do anything. I have never been so angry or see my Dad cry. He said he just wanted to die and wouldn't even eat anymore. And they threw a fit when we went and got him. Said it was against medical orders. AWFUL PLACE AND UNCARING PEOPLE IN THAT GOD FORSAKEN PLACE!!!

This facility is AMAZING!!
We have a friend who has been staying here for about a year now. He LOVES it there!! He says it is a very warm place to be!!
Yesterday afternoon, we were visiting, and also were invited to the “Happy Hour” they provided. There was live music and dancing, and the room was filled to the brim!! It was SO much fun for everyone, including myself too!!
We did have lunch there, the choices offered on the menu really surprised me, and the food was outstanding!! The dining room, food and service made us feel like we were in one of the finest restaurants in town!!
You folks really have, providing a place for Seniors, down!!
The amount of activities, the “field trips”, and so many other amenities you provide really assures me that our friends is in the right place!!

I moved my very elderly (late 90s) mother from Brookdale to The Peaks in fall of 2023 because there were so many problems with getting the staff at Brookdale to implement my mother's (very minimal) care plan. I read the great reviews of The Peaks and when I toured the facility I was impressed. It's gorgeous. Do not let the beautiful facility fool you. In the six months since my mother has been at The Peaks they have failed repeatedly to do simple, but absolutely necessary, things such as helping her with compression socks that are ordered by her primary care provider and written in her care plan. Even though we are paying for medication management and supervision, they have left medication in her room for her to take "on her own" (which she didn't). According to their own policy, they are to notify me two weeks prior to her medication running out so I can order it from CostPlus Online Pharmacy so her medications are delivered on time. Instead, I was notified today that she only has three days medication left. When I asked what went wrong I was told by their head of nursing "we're only human" (the same thing I was told when they left the medication in her room ), that she didn't know what went wrong with notifying me, and that she doesn't speak in terms of never and always so she cannot tell me this mistake won't happen again. She did promise to meet with staff and make it "Barney simple" - whatever that means. Also, if you're considering having your elder loved one live here, or if they already are, please check how much Saliba's pharmacy is charging. I was told when my mother moved in that I had to use Saliba's (that is not true) and when I found out that medications I can order online for $15.00 for a 90-day supply were costing my mother over $180/mo I just about flipped. Nothing like price-gouging our elders on their necessary meds. The nurse today told me that of the over 120 residents at The Peaks less than 5 use online pharmacies and the rest use Saliba's. I know it's hard, but please do your due diligence and price compare for medications. Saliba's is NOT your best option. I know there are limited choices for elder care in Flagstaff, but please don't let the glitz of The Peaks fool you into thinking you or your loved one is going to get consistently great care. That has not been my experience at all.

My mother is at The Peaks and I have been please with the care she has received. Every senior home will have it's problems here and there, but everyone I have interacted with has been polite and helpful.

About this data
How to read this page
Star ratings vs. Care Quality Score: The Google star rating reflects visitor and family opinions collected from public reviews. The Care Quality Score is an independent metric derived entirely from federal inspection data — the two measure different things, and comparing them side-by-side gives a fuller picture.
Where this data comes from
All clinical data on this page originates from the Centers for Medicare & Medicaid Services (CMS), the federal agency that certifies and inspects every Medicare- and Medicaid-participating nursing home in the United States. Reviews are imported verbatim from public Google business listings.
