My Mom is here with a hip fracture and had many comorbid conditions including bad heart, severe anxiety and dementia. The pluses here are the staff is very friendly and smiles a lot. The therapy staff are very good and sweet. The nurses have all been good but there is a little bit of miscommunication and the rooms are very tight/small which I know is the norm for these skilled nursing rehabs. Most of my concerns were addressed in all. Special shout out to Kim, (nurse manager), Sara (restorative nursing assistant), Krys (OT assistant), and Mikayla (speech therapist), Jay and Antoinette CNA’s, and Tauna and Amanda from nursing who always listened and addressed my concerns. The place is clean but the food is not good for my mother who has a very restrictive diet . My father decided to move my mom out earlier than expected because she is so overly anxious and severely depressed and needs her home asap to heal. Overall I am pleased with the care my mom had but I recommend name tags to be worn by staff and also diaper checks more frequently when people have bowel and bladder incontinence. Thank you everyone at Citadel for allowing me to participate in my mother’s care and transfers when there due to my experience. I believed it helped her a lot but unfortunately I had to go back home to California. I pray the transition home goes as smoothly as possible due to the short notice of request to go home (the facility and myself found out today my father’s request). Also the occupational therapist Beth was very kind to help me change my mother’s diaper before we got her in the wheelchair the first day and did not run for a nurse to change her. I was very impressed with meeting the physiatrist’s assistant Geoff who was very knowledgeable and understood that we did not have high hopes for my Mother’s physical recovery because she was on hospice at home prior to her fall and resultant fracture. I would recommend this facility for people who have need for rehabilitation but are not confused and able to understand goals setting. Thank you all from the bottom of my heart.

First visit and the facility appeared to be well run and clean. Courteous staff. Attentive nurses. My sister was calm and relaxed so I can assume she is fine. I’ve been to other facilities that were not impressive.

This place was very dirty. As soon as you walk in, it smells like dirty diapers. Most of the staff is rude The first nurse i spoke to when we checked my mom in was laughing at her because she couldn't remember what year we're in. My mom had a seizure and hit her head and was not cognitive. I feel that was very disrespectful. Also, the food is horrible! And the fact that they only bathe patients twice a week is nasty.
