I had an excellent experience with Sierra Vista BHC over the past few months. The facility is very well managed and supported by a caring, professional team. Ify, Tiffany, and Genevieve were all exceptional. Ify led the Social Work Department with great professionalism and was extremely helpful during the relocation process—quick to respond and always supportive. Tiffany, my main point of contact, provided outstanding communication and follow-through from start to finish. Genevieve (RN) managed treatment and medication coordination with patience and attention to detail, even though changes in medication forms. The nursing and support staff, including Michael (RN), Hong (RN), and Lenisha (Social Worker), were always kind, welcoming, and patient during my visits. Although the facility building is older, it’s well maintained, and the visiting room is always clean and pleasant. My only suggestion for improvement would be to make it easier to communicate directly with the doctors. Overall, Sierra Vista BHC is a well-run facility with an excellent, compassionate team that truly cares about their patients. Highly recommended.

My Uncle was at this facility for years and when I called to see about visiting him once they started to reopen the world (after stay at home order and such) I was told they had no one there by that name. Only to find out that he had been released (so they say) and they can't release any information due to HIPPA. My Uncle has now been missing for almost 2 years and my family is at wits end as we don't know where to begin to look for him. They can't even give us the city in which the board and care is located that he was supposedly sent to. Please pray for my family as we are still hoping he is out there somewhere safe Update: Now well into year 3 of my Uncle missing from this facility with no help from the police or anyone. After reading these new comments it is really disheartening to think of what has actually become of my Uncle.

About this data
How to read this page
Star ratings vs. Care Quality Score: The Google star rating reflects visitor and family opinions collected from public reviews. The Care Quality Score is an independent metric derived entirely from federal inspection data — the two measure different things, and comparing them side-by-side gives a fuller picture.
Where this data comes from
All clinical data on this page originates from the Centers for Medicare & Medicaid Services (CMS), the federal agency that certifies and inspects every Medicare- and Medicaid-participating nursing home in the United States. Reviews are imported verbatim from public Google business listings.
