The care at city creek is just amazing. The CNAs are marvelous and the food is great. This a 5 star facility for sure. I'd like to give a shout out to Lito the CNA and Nick the administrator for always being willing to help. He communicates with all us residents and I think it's a great thing.

Kaiser recommends this rehabilitation center. The facility is very clean, and the staff are extremely friendly. Kudos to Heather and her team for assisting my mom in recovering and reaching her original health. Jean, the nurse case manager, was excellent. She was professional, compassionate, and highly responsive. She kept us updated every step of the way and genuinely advocated for the patient. We are deeply thankful for her care and support. This is a great place to entrust your loved ones who require rehab.

About this data
How to read this page
Star ratings vs. Care Quality Score: The Google star rating reflects visitor and family opinions collected from public reviews. The Care Quality Score is an independent metric derived entirely from federal inspection data — the two measure different things, and comparing them side-by-side gives a fuller picture.
Where this data comes from
All clinical data on this page originates from the Centers for Medicare & Medicaid Services (CMS), the federal agency that certifies and inspects every Medicare- and Medicaid-participating nursing home in the United States. Reviews are imported verbatim from public Google business listings.

Data is a starting point, not a final answer. We encourage families to visit facilities in person, observe daily routines, and speak with staff and residents. See our guide: Questions to Ask When Visiting →
