Dear Angelina,

I am the daughter of your resident, Mr. Fully S. at Greenhaven Healthcare Center in Sacramento, California.

I am immensely grateful to you and everyone at Greenhaven who have provided compassionate care to my father ever since he was admitted in December. My father went through his life’s darkest and toughest moments for intensive antibiotics and stage 3 sacral bed sores wound treatment. You and your team have remained a beacon of hope and care for him. I hope my father will continue receiving medical care, physical therapy, and nursing care at Greenhaven. I visit my father every now and then since I am from out of town. It is gratifying to see his health being and wound care has been improved slowly.
Your kindness and compassion make Greenhaven a better place for my father.

Greenhaven management team and nursing staff are truly the unsung heroes of healthcare and elder care!

Graciously yours,

Virginia

This was the worst experience of my life. August 2025 after knee replacement surgery and staying in the hospital for a week, I was sent to this facility which I picked because I recognized the area as being close to my family. I had no idea how bad it would be.

Problem #1 it stank upon entering you could smell how bad it stank throughout the entire facility.
Problem #2 the transport people were willing to dress the bed they had for me because it was not ready. Then when someone finally came and put a sheet on the bed it had holes, no blanket just a sheet with holes.

Problem #3 All the furnishings were at least 20 years old.
Problem #4 None of the staff that came into my room were wearing name badges or name tags,
Problem #5 People were screaming down the hall
Problem #6 The curtains that separated me from the other person in the room were so dirty that you could see the dirt.
Problem #7 No one had any idea of who I was or why I was there.

The bottom line is that I felt so extremely uncomfortable and afraid of what would happen to me if my husband left me there.

I would not recommend this place to my enemy. I understand that facilities are often understaffed unfortunately that should not stop them from running efficiently.

NEVER GO TO THIS PLACE AND DON'T LEAVE YOUR FAMILY MEMBERS THERE.

Because I had Gout and couldn’t stand on my feet and walk very long. I was discharged from Greenhaven Healthcare Center to go to my house where I happen to live alone, I would have had no one to help me get in my wheelchair to maneuver around my house, I also would have had no one to help me get to the toilet , to make food, or to bathe. I was encouraged by the Advice Nurse at Kaiser to go back to the ER at Kaiser and to be readmitted. I believe it was in retaliation for me complaining about the portion size of dinner. Dinner was served at 5 PM and you had nothing else to eat until breakfast at 7 AM. My insurance was paying $625 a day for me to be at this facility. I had to ask for permission in order to Door-Dash something over to the facility to eat for my dinner. It was really a shame that with the amount of money that was being charged to my insurance company for me to be taken care of at Greenhaven Healthcare Center, that I would have to pay for my own dinner not once but twice. Theses are pictures of two of the dinner meals. After speaking to the chef, she said to just ask for snacks when I was hungry again. I Thank her for The offer so that I wouldn’t have to spend more money for dinner.

I can't say enough great things about this skilled nursing facility. Every single person we worked with this, from intake, to case management, to discussions of discharge has been wonderful. They are genuinely compassionate individuals who strive to provide the best care. The medical assistants, nurses, and administrative folks are all so kind and really show patients attention and compassion. Special thank you and appreciation goes to Dr. Masocal who has been taking care of my husband's aunt. She is communicative, thorough, and truly wants what is in her patients' best interest. Also, one more big thank you and our deepest appreciation goes to the discharge planner, Angelina. This woman truly juggles it all and manages caseloads with highest level of attention, detail, and you can tell she is very experienced. If you have Dr. Masocal or Angelina assigned to your loved one's care, you are in amazing hands. Thank you Greenhaven Healthcare for the care and compassion you provide to our community's loved ones.

About this data
How to read this page
Star ratings vs. Care Quality Score: The Google star rating reflects visitor and family opinions collected from public reviews. The Care Quality Score is an independent metric derived entirely from federal inspection data — the two measure different things, and comparing them side-by-side gives a fuller picture.
Where this data comes from
All clinical data on this page originates from the Centers for Medicare & Medicaid Services (CMS), the federal agency that certifies and inspects every Medicare- and Medicaid-participating nursing home in the United States. Reviews are imported verbatim from public Google business listings.
