I was a patient in Room 117, Bed 2, and I’d like to express my deepest gratitude for the exceptional care I received. My nurse, Chandanpreet Kaur, provided truly outstanding care throughout my stay. She was consistently professional, kind, and attentive, always wearing a warm smile and maintaining a positive attitude. Her dedication to her work was evident in everything she did. Chandan is a valuable asset to the nursing staff. My two-month stay at Autumn Lake Healthcare felt like being at home. The facility is immaculately clean, and the atmosphere is warm and welcoming. It truly is a wonderful place for healing and recovery. I returned again at the end of November for another recovery stay, this time in Wing B. During that visit, I was cared for by Reneisha Bryan and Anika Paul, who attended to me with so much care, patience, and attention. They were both wonderful nurses who made me feel safe and well cared for. Even though I was no longer in her wing, Chandanpreet Kaur continued to show the same level of professionalism and compassion by coming to check on me. That meant so much to me—it made me feel comfortable and reminded me of my previous stay, which I truly appreciate. Everyone at Autumn Lake Healthcare is efficient, compassionate, and dedicated to their patients. They ensure patients are well groomed, properly fed, receive physical therapy, and are treated with dignity and respect. I highly recommend this center. I would even want to live here in my elder years because I know I would be taken care of to the fullest. Thank you Lorraine my case manager

I was disappointed that no one followed up with me when I reported my Dad losing his dentures while in the care of Autumn lake. I left text messages and phone messages with Raya Perez (new administrator) but she never contacted me to resolve the problem. My Dad is too old to get new dentures so now has to eat pureed food which is a big adjustment for him.

My father was there for two months after fracturing his pelvis. The staff was very kind and even loving with my dad. The food was good and the facilities were clean. I especially want to thank the nurse Chandra and aides Dupree, Jacqueline, Daniele and Raj. The physical therapist, Karina was very good with my father. They were all hard workers with a great attitude. I would definitely recommend!
