Very concerned with this place. In recent months, rooms are filthy, as if grown adults don't know how to use a mop and a broom. Food is horrible. Supposed "chicken alfredo" was served here a week or so ago; it was just alfredo sauce in a bowl with a side of vegetable and slice of bread. Pda between staff. Washing machines need fixed. Staff have attitudes 24/7, even to residents. Call lights never answered. It disappoints me to write this review, as this was once a really good home, but the mistreatment here is not normal. Administration needs to wake up and take care of these problems. Until everything is shaped up, I wouldn't recommend registering your family.

From personal experience I would never take a loved one here. Look into home health before here!

This use to be a well organized place. They use to care about there patients. The new owner have been shifting all the Aides different units. Especially in the the unit for memory. Don’t the management know these people need the person caring for them. They would be more productive and calmer. What a shame

This home was formerly known as Heartland. It has been sold several times and it no longer is worthy of the once top reputation it once enjoyed. My grandmother was a resident in the memory care unit from 2017 through Jan 2022. Initially, we were happy with her care as the staff was loving and attentive. We saw progressively greater staff turnover with each sale and fewer activities for the residents and much less housekeeping. Even the quality of the food deteriorated from home cooked meals to something very institutional. The more impaired she became due to her dementia the more apparent it was that the staff was not really trained in dementia care but some were naturally better at it. We were in the process of moving her to another home, but then she passed. This used to be such a reputable warm home that it saddens me to write this review. If you are reading this, I hope you have the power to change the environment as an administrator or as a family have the ability to choose another place for your loved one.

About this data
How to read this page
Star ratings vs. Care Quality Score: The Google star rating reflects visitor and family opinions collected from public reviews. The Care Quality Score is an independent metric derived entirely from federal inspection data — the two measure different things, and comparing them side-by-side gives a fuller picture.
Where this data comes from
All clinical data on this page originates from the Centers for Medicare & Medicaid Services (CMS), the federal agency that certifies and inspects every Medicare- and Medicaid-participating nursing home in the United States. Reviews are imported verbatim from public Google business listings.
