Extremely low-quality food with no standards. Food not to temperature., vary small menu in it's entirety (same food may be recieved weekly), activity staff help themselves to food meant for residents, disrespectful demeanor of staff toward residents which is what I consider abuse. Out of health & comfort items for long periods of time such as shaving cream and tooth paste. Nurses come to start of shift at their leisure and not at scheduled times. Residents are without care when their nurse is not on duty, this is a serious issue. Rights are not respected and briefed to along with residents. (i.e.- day passes that are allowed to residents are not given notice of such privileges for the convenience of facility personnel and can only be restricted if documented in theirvclinical file otherwise this is considered 'confinement' under Illinois State Statute 210 ILCS 45/ of the nursing home act.

This place really should be shut down, my husband is being singled out and when he first arrived, legitimately nobody gave him papers to sign, nobody explained the rules and regulations and all we got told was that the case manager or whoever did my husbands intake is usually too busy, well if your continuing to want that paycheck then I suggest you step it up and if you can’t handle a simple intake correctly then I really don’t think you should be working there. But then again in places like this, money is the only thing that’s on these “nurses” minds. Didn’t go over any of my husbands needs. And my husband has voiced these concerns multiple times and all he gets is “I didn’t know that” or “I don’t know what to do” “well that’s not our policy” how is my husband supposed to know if you guys are the one who didn’t tell him and to pin it as his fault and take away his passes all bc your employees don’t wanna do their jobs????????? He hasn’t even signed one piece of paper and he’s been there over a week. It’s so unprofessional and honestly just plain out incompetent. If things don’t get better in the next few days I think I’m just gonna go to the upper hand. I’m tired of places like these being in the care of people who can’t advocate for themselves. And btw smoking isn’t necessarily but if your gonna allow cigarettes then a nicotine vape should be allowed but apparently they are not allowed. So if one nicotine device ain’t allowed, no nicotine should be allowed (which means no cigarettes) Plain and simple. Make it make sense please management. Now it’s on you if your gonna show compassion to your residents or if your gonna ignore. I will come back here to give an update.

Very horrible. No clear instructions if you are a visitor.

The front staff security, Carmelita is a sourpuss who treats visitors and residents like they have no business in her domain. I went to visit my friend and had pizza and gifts for Christmas and she wouldn't let me put the pizza down on the counter. She expected me to sign in without a pen. Every single time I come there she has attitude. If you don't like your job then don't take it out on the residents and visitors. Smile, it's not that hard to do. 😀 Remember, the residents live there and people who come there are bringing them encouragement and companionship. Your precious counter can be wiped down.

About this data
How to read this page
Star ratings vs. Care Quality Score: The Google star rating reflects visitor and family opinions collected from public reviews. The Care Quality Score is an independent metric derived entirely from federal inspection data — the two measure different things, and comparing them side-by-side gives a fuller picture.
Where this data comes from
All clinical data on this page originates from the Centers for Medicare & Medicaid Services (CMS), the federal agency that certifies and inspects every Medicare- and Medicaid-participating nursing home in the United States. Reviews are imported verbatim from public Google business listings.
What to do next
Data is a starting point, not a final answer. We encourage families to visit facilities in person, observe daily routines, and speak with staff and residents. See our guide: Questions to Ask When Visiting →
