I just want to say that when I first arrived at the facility, I wasn't sure if it was going to be a good fit for my mom who was needing rehab, after a lengthy hospital stay. Well, within a few hours, I knew that she was in the right place. All of the staff were so good with her, and she didn't have to wait long to have a need met, which is what you want for your 94 year old mom. Food is important to my mom, and she loved the food. It was all of a homemade type of food and every meal arrived hot and was very good. That's important when you are recovering! The first night she arrived, Nurse Jennifer was preparing to get her all checked in, and realized the time of day (late evening) and knew the kitchen would be closing soon for the day. She put everything on hold and made sure my mom had a hot dinner before moving on to getting her checked in.
The physical therapy and occupational therapy was also good and they worked with my mom 5 days a week, and sometimes on the weekend too. The only thing is after PT & OT patients are returned to the room and unable to move around without help for their own safety, did I mention she is 94.
I guess I would say don't judge a book by the cover, SLSCR is clean, but very dated, ( my mom said the bed was very comfortable, just old like her), and could use some upgrades, but I will take an older facility with caring staff and good food over a newer facility and staff that doesn't care for your loved ones needs.
After two weeks I decided it was time to get her home, and they helped get her set up for home health care for continued rehabilitation, which went very smoothly. She is now recovering even more, but if she needs rehab or nursing care, I will definitely look at SLSCR for her needs.

About this data
How to read this page
Star ratings vs. Care Quality Score: The Google star rating reflects visitor and family opinions collected from public reviews. The Care Quality Score is an independent metric derived entirely from federal inspection data — the two measure different things, and comparing them side-by-side gives a fuller picture.
Where this data comes from
All clinical data on this page originates from the Centers for Medicare & Medicaid Services (CMS), the federal agency that certifies and inspects every Medicare- and Medicaid-participating nursing home in the United States. Reviews are imported verbatim from public Google business listings.
