At first I was not sure about Oak Park Place, because of what I was told by others in the public but what places don't have some problems to work on. My mom who has alzheimer's moved in Oak Park place in Janesville in April of 2025. The memory unit is very clean, and apartments are cute. Mom adjusted better then I thought she would. Mom came from living in a little log cabin her and my Dad built. After Dad died Mom was alone and showing alzheimer's. It was a tough decision to put mom in a memory care but now I can sleep at night knowing she is safe. Now she has friends to talk to everyday and gets the care she needs but the big plus is none of the family worries about her being alone in her house in the country anymore. She is now safe and we can all sleep at night knowing she is safe and not wondering somewhere. If I see anything I don't like or I'm concerned about , I go talk to staff with my concernes and its taken care of. The head nurse Melissa is great with mom. Mom just loves her and Melissa is great of letting me know how Mom is doing. I can also talk to Melissa about concernes even advise on how to deal with mom and her Alzheimer's. The rest of the staff who are nurses is also great with mom, Savanna, Lori, Kyle, Kristen, Patty, Chris and so many others. Sorry if I missed anyone. Lori and Betty are the people to talk to about questions about Oak Park Place on which is best for your love ones., whether it be independent, assisted, or memory care apartments. I wanted mom to be in assisted living but they suggested memory care. I did agree when mom said to me " I can leave here antime right?" I said "No mom you can't ". I was afraid that would happen in assisted living and she would leave and get lost. Lori and Betty are so professional and really do care. Always have the time to answer your questions. If you need memory care for your love ones, get a hold of Betty for a tour of memory care or assisted living or independent living. The place is beautiful and very clean. The front desk people greet you with a smile and are just wonderful. Very helpful when you have questions. At this point, I'm very happy with Oak Park Place. Thank you Oak park staff for taking care of mom. Price is lower then most places. Their rent includes meals and that's 3 per day and snacks. Laundry, is also included. The biggest plus is your love ones are safe and taken care of, and that's a big plus for me and my family.

After a long, beautiful life as a family man, Korean War veteran, father, grandfather, and great grandfather, it is an absolute shame that my grandfather died in this facility. The room was never cleaned (until it was demanded) and his blankets always dirty. The long hallways were virtually empty of any helpful staff and the inexperienced teenagers working there were of little use. We had to question why getting him food or water needed to be a chore - not getting him to eat, but actually having it available. We had to question why they didn’t have adequately trained staff to insert his catheter. We were mortified to find out he fell not once, but four times the two weeks leading to his final breath- he was just found on the floor after an undetermined amount of time because nobody checked in on him at a regular interval. Four falls at his age meant that his final days were painful if not accelerated. When I called and got through the Laurie the admin (whose first question she asked when she picked up was literally “What are you trying to sell me?”), she couldn’t guarantee the safety of my grandfather claiming it would violate HIPAA - which is absolutely not true. Laurie also seemed to be surprised to see my grandfather’s room empty after his body was removed, which tells the whole family that she is unaware of deaths occurring in her facility. A last twist of the knife occurred a week or so later when we received a card in the mail from the facility- not sending condolences of any kind- but a card wishing us a “Merry Christmas.” Choices may be limited and end of life care is unimaginably difficulty to navigate for so many reasons- if you can avoid it, do not allow your loved ones to stay here.

My mom was there and the nurses were nice, food was good but her room was always dirty, the communication to us was not good and we tried. She took a bad fall because they did nothing about her high blood pressure. Maybe tney showered her once a week which is not enough. The physical therapy was great. Her meds got messed up a few times. Doctor didnt come around much also.

I have been taking my students to socialize with the seniors at Oak Park for quite a few years now. My students love interacting with them. To see the intergenerational relationships forming is invaluable. The director, Nicole Pfundheller, is amazing. She goes above and beyond to make sure that our students are well taken care of. They love to play games sing and have snacks together. The students look forward ro coming every week!!

About this data
How to read this page
Star ratings vs. Care Quality Score: The Google star rating reflects visitor and family opinions collected from public reviews. The Care Quality Score is an independent metric derived entirely from federal inspection data — the two measure different things, and comparing them side-by-side gives a fuller picture.
Where this data comes from
All clinical data on this page originates from the Centers for Medicare & Medicaid Services (CMS), the federal agency that certifies and inspects every Medicare- and Medicaid-participating nursing home in the United States. Reviews are imported verbatim from public Google business listings.
