My loved one is staying in the newer part of Autumn Lakes for over a month of rehab and IV antibiotics. This place is amazing. It has a wonderful atmosphere, it is very clean, quiet and the rooms are large with a cozy feel. I visit every day at different times mostly in the evening and every staff member I have met has been nothing but extremely kind and so caring to my aunt and to me. The level of care she is getting is top notch, from her room care, IV’s, physical therapy, food, and other daily needs, it all has been outstanding. My aunt herself has said how happy she is staying there. A special shout out to the wonderful evening nurses and staff who I see daily and make it such a welcome warm environment. Thank you Autumn Lakes for all you do!

One star is too much for this place. My sister was in this facility for less than a week and I had to call 911 to have her taken to the ER. She had a blood infection from a neglected PICC line and unsanitary practices by staff. In the short time she was there, there was only 1 day where actual facility CNA's were present. They operate through agency placement apps!!! On day 1 her bed did not raise or lower with the control. We requested it be fixed a minimum of 4 times. That never happened. There was dust about a 1/2 thick on every blade of her ceiling fan. The requests to clean that went unanswered. The floor was never cleaned, rather messes were wiped up with paper towel. We also put in several requests to have the toilet fixed as it ran constantly. Again, the work was never done. Call button requests, if responded to at all, were answered by a woman who would say every time "I'll tell someone". 80% of the time "someone" never came. As a result, she fell several times during the night trying to get to the bathroom herself. Their solution was to throw an adult diaper on her. She was left laying in pools of feces as not 1 staff could properly place a stoma appliance. Staff had no idea how to run a nutrition bag through the PICC line. On her last night there, it was not running and the machine beeped all night. She couldn't get to her call button as it had fallen on the floor. However, this indicates that not 1 person checked on her all night!! On another day, I personally witnessed the connection hose from the nutrition bag to the PICC line laying on the floor and the milky fluid was running all over the floor. The RN came in, picked it up and plugged it back in the port WITHOUT first cleaning it! On her last day there, my younger sister witnessed an RN wiping my sister's already raw stoma with wipes that contained some type of cleansing chemicals which burned her stoma and the surrounding skin. If you have a loved one, please do not place them here. We were told by a hospital social worker that this place was our only option. We have subsequently found that to be false.

Annette, the director, has yet to return phone calls on issues in her facility. She seemed interested in my list of concerns until it was time to face them. I am disappointed in the directors professionalism. They are shortstaffed currently check their website for the list of departments short staffed. I wouldn’t send your loved ones here anytime soon. 8/14/2025
