A family member stayed here for rehab and during their stay we were told that they were doing such a good job and improving so much, that the insurance ended up cutting their stay short.. but upon returning home they could barely walk without assistance, use the bathroom without assistance, and had a new wound that no one at the facility knew about it. The discharge paperwork did not reflect the reality of the situation and my family member was unwell and unstable on their feet, it was incredibly unfortunate. After a week at home, my family member fell and passed a day later. I don’t know what happened to lead to their abrupt discharge but it was all and all a series of unfortunate events.

Terrible, chairs broken, residents don’t have comfortable recliners in their rooms, one nurse only available during the day, no nurse in evening or weekends. Who gives the meds? Not the nurse. No one answers the phone. One staff member for entire floor. One would think $10k a month would offer decent care for loved ones.

My grandmother’s stay at Lindengrove was from late January 2025 to March 19th, 2025 (the day she passed away). She was placed on hospice due to being diagnosed with rectal cancer. Grandma was 93 years old. Being the primary caregiver for my grandma, it was very important for me to place her somewhere that I knew she would be well taken care of. She meant the world to me and I needed to make sure she was in good hands. My father and I took a tour of Lindengrove and were greeted with Anna Kulinski. From day one, Anna has been nothing short of AMAZING!!! I do not have enough words to thank you for everything you did for my family and I!!! I am forever grateful and I believe you know this. ❤️ Placing grandma in a facility was a difficult decision and not taken lightly… after time grandma would communicate with me about how great the staff were to her. I would continuously ask her how things were going and she would reassure me that things were fine. It brought me comfort. There were some minor mishaps along the way, but the staff were always willing to help out and fix any issues. Thank you to Suzy as well for all her help along the way. Suzy was the social worker who assisted when grandmas dentures were lost. She went above and beyond to make sure the issue was resolved. Thank you so much!!! And although grandma wasn’t able to enjoy a set of new dentures we are forever grateful for all the steps you took to try to make that happen!!! I visited my grandma daily, along with my father, and we were truly happy with the way grandma was taken care of during her final days. With time grandma became more and more comfortable with Lindengrove and that brought us a sense of comfort I can’t explain. She never wanted to leave her home so placing her was not an easy decision but necessary for her safety and the level of care she required. Anyone who knows me, knows how much I loved my grandma and would do anything in this world for her!!! Thank you for treating her so well during her final days!!! Thank you to all the staff on D Unit who loved her and treated her so well!!! We saw it and she expressed it to us!!! Thank you to Commonheart Hospice staff as well!! We appreciate you alI!!! I definitely recommend Lindengrove for a loved one who is in need of care.

My mother-in-law is 92 and has advanced memory loss. She declined very quickly and we needed to move her to a community that could provide appropriate care for her. We had investigated Linden Grove New Berlin and other communities about a month prior to needing to move Mom. We were so happy with the Linden Grove environment, available services, that we were treated so respectfully, and that Linden Grove could accommodate our urgent need. We were able to move Mom very quickly at the end of December 2024 and are so pleased with how she transitioned easily and without much agitation or worry. The staff truly cares about each resident, making their final journey loving, dignified and respectful. The nurse and health care providers keep in close touch with us, making sure Mom stays as healthy and happy as possible. Although it's never ideal or easy to transfer someone you love so much to a memory care community, we really feel we made a great decision with Linden Grove. Meals are great, activities are plentiful, cost is appropriate and we've been fortunate to meet and get to know most of the other residents. Nothing is every perfect, but Linden Grove is among the best for caring for loved ones.

How to read this page
Star ratings vs. Care Quality Score: The Google star rating reflects visitor and family opinions collected from public reviews. The Care Quality Score is an independent metric derived entirely from federal inspection data — the two measure different things, and comparing them side-by-side gives a fuller picture.
Where this data comes from
All clinical data on this page originates from the Centers for Medicare & Medicaid Services (CMS), the federal agency that certifies and inspects every Medicare- and Medicaid-participating nursing home in the United States. Reviews are imported verbatim from public Google business listings.
